GOODMAN SET UP A MEETING W/ MANAGER, REGIONAL MANAGER, MAINT. SUPERVISOR FROM PHOENIX, FOR NEXT DAY TO PUT A PLAN TOGETHER ON IMMEDIATE REPAIRS, PLANS ON WHAT IS TO BE DONE TO COMPLETE SYSTEM PIPING, ALL AGREED ON MEETING 5/20/05. ONCE THIS MEETING WAS IN PLACE, CHECKED W/ BERTS PLUMBING WILL STAY ON SITE TO CONTINUE MAKING REPAIRS AS NEEDED. STILL PUMPING OUT WATER ON FLOODED UNITS, NOT ANTICIPATING TO DO SHUT DOWN OF UNITS AT THIS TIME DUE TO GETTING INTO COOLER CLIMATE OF EVENING W/ BREEZE AND BERTS PLUMBING STAYING ON SITE TO MAKE ANY AND ALL REPAIRS AS NEEDED. GEORGE MAINTENANCE PERSON MENTIONED THAT COMPLEX HAS HAD ABECUS & BERT'S PLUMBING AND ALSO MAINTENANCE HAS FIXED LEAKS THEMSELVES. SHOWED ME THREE AREAS WHERE THEY HAVE LEAKS IN OR AROUND THE COMPLEX. MENTIONED ONCE FIXED IS COMPLETED THE CHILLER SYSTEM WILL BE TURNED BACK ON. 163-B INSIDE, TURNED SYSTEM OFF - TO UNIT 711. 153-B INSIDE TURNED SYSTEM OFF. 63-B OUTSIDE LEAK, PUMPED WATER LEFT SYSTEM ON. - WALKED COMPLEX AS WALKING FOUND SEVERAL BELCONIES, RAILINGS DECKS TO BE ROTTED, POSTS ROTTED AND NAILS STICKING UP OUT OF 2X8 DECKING. - FOUND ELECTRICAL BOXES THROUGHOUT THE UNIT TO BE KNOCKED OVER AND BROKEN, EXPOSED WIRING CONDUIT GOES UNDERGROUND. - FOUND POOL TO BE DRAINED AND UNDER CONSTRUCTION, GATES LOCKED. BUILDING #7 & ALL BALCONIES POSTS TO BELCONIES HAVE ROTTED AT BASE. - RAFTERS SEEM TO BE OVER SPANNED (FLOOR FIRST). - LEDGER IS PULLING AWAY FROM STRUCTURE. - BEAMS ARE ROTTEN PAINT PEELING & FLECKING. - 2X8 DECKING - BOARDS ARE PULLING AWAY FROM JOINTS, TWISTED, PEELING & FLECKING. BRIDGETT GONZALES #406-2030 UNIT #578 LAUNDRY ROOM: SOUTH WALL HAS HOLES IN WALL. WOOD INSIDE WALL IS COMPLETELY ROTTED. PAINT PEELING OFF. CEILING IN SAME AREA FALLING DOWN. ONE GATE WAS LOCKING SELF CLOSING HARDWARE, BOTH GATES WERE LOCKED WHILE POOL WAS BEING RE-GUNNITED. POOL FOR AREA A WAS OK, (3) GATES HAD SELF CLOSING & LATCHING HARDWARE. THERE WERE NO OPENINGS LARGER THAN 4". POOL B DID HAVE (2) OPENINGS WHERE WROUGHT IRON HAD BEEN BENT & BROKEN IT WAS CLOSED BY CHICKEN WIRE WIRED TO THE FENCE TO AVOID ANYBODY ENTRY TO THE POOL AREA. HAD REBECAH CONTACT REGIONAL MANAGER TO FIND OUT IF PERMITS HAD BEEN PULLED FOR WORK IN PROCESS FOR POOL AND RE-STUCCOING OF THE UNIT A. 06/22/2005 INSP - FOLLOW UP CODE ENFORCE GGOODMAN Approved 6/22/05 VISITED SITE DUE TO CALL RECEIVED FROM UPSET TENANT CONCERNING THE CHILLER SYSTEM BEING DOWN AGAIN AND NOT HAVING ANY AC. MET AND TALKED TO FLYNN JANISSE REGIONAL MANAGER FROM PHOENIX WHO WAS ONSITE TO ACTUALLY MEET WITH NEW OWNER WHO WAS IN TOWN 6/21- 6/22 TO EVALUATE PROPERTY AND DETERMINE THE PROGRESS AT MT VIEW APARTMENTS. AS DNR INSPECTORS ARRIVED ONSITE, YORK AIR CONDITIONING WAS LEAVING THE BUILDING AFTER COMPLETING SERVICE TO CHILLER SYSTEM. SPOKE TO FLYNN, ASKED IF SYSTEM WAS STILL DOWN, SHE SAID NO THAT YORK AC WAS THERE AND COMPLETED THEIR WORK AND HAD SYSTEM UP AND RUNNING AGAIN THAT IT HAD BEEN DOWN FOR A COUPLE OF HOURS. SO BY THE TIME OF OUR ARRIVAL THE SYSTEM WAS WORKING. ASKED FLYNN WHAT THE PROGRESS WAS OF ALL THE ISSUES AT THE SITE, SHE MENTIONED THAT STUCCO PERMIT HAD BEEN OBTAINED, THAT IT TOOK SOME TIME TO FIND AN ARCHITECT THAT WAS WILLING AND SIGN OFF ON EXISTING WORK, ONE WAS FOUND, PRINTS DRAWN, AND PERMIT OBTAINED, AND THAT WORK HAD ALREADY BEEN SIGNED OFF ON. FLYNN THEN MENTIONED THAT THEY HAD RECEIVED BIDS FOR REPLACING THE BALCONIES, THAT THE OWNER HAD JUST SIGNED THE BID PAPER WORK AND THAT KOWALSKY CONSTRUCTION WOULD BE THE COMPANY PERFORMING THE WORK. EXPLAINED TO FLYNN THAT THE MOST IMPORTANT PART OF THE PUZZLE WAS THE PLUMBING AND ASKED WHERE THEY WERE AT WITH THE PLUMBING BIDS? FLYNN MENTIONED THAT THE PLUMBING BIDS HAVE BEEN ADDRESSED AND THAT AT THIS TIME THEY HAD RECEIVED 2 BIDS AND WERE WAITING FOR A 3RD AT WHICH TIME THEY WOULD BE MAKING A DECISION AND THEN MOVE FORWARD. SHE REITTERATED THAT THE FUNDS WERE STILL AVAILABLE TO COMPLETE THE PLUMBING PHASE. I ASKED WHAT THE CAUSE FOR REPAIRS WAS, SHE MENTIONED THAT WATER HAD LEAKED ONTO THE CHILLERS MANIFOLD, HAD GOTTEN INTO A DISTRIBUTOR CAP, CORRODED AND BURNED UP THE DISTRIBUTOR. SAID SYSTEM WAS DOWN MINIMAL AMOUNT OF TIME. DNR INSPECTOR C-16 WAS SASTIFIED WITH THE PROGRESS MADE BY THE BERNARD/ ALLISON MANAGEMENT COMPANY, WHILE REALIZING THAT IT DOES TAKE SOME TIME TO GET BIDS TOGETHER FOR A PROJECT OF THIS SIZE. AFTER LEAVING THE SITE I CALLED KOWALSKY CONSTRUCTION , SPOKE WITH MANAGER MELLISA WHO CONFIRMED THAT THEY HAD GEVEN A BID TO BERNARDL/ALLISON FOR THE BUILDING OF THE NEW BALCONIES. MENTIONED THAT SHE WAS HOPING TO RECEIVE THE SIGNED DOCUMENTS IN THE NEXT FEW DAYS, AND WHEN THAT HAPPENED WOULD EXPECT WORK TO BEGIN NEXT WEEK.WEEK OF 6/27/05. WORK MEANING HAVING THE PRINTS DRAWN AND SUBMITTED TO DSD. CONFIRMED THAT THEY WOULD BE OBTAINING PERMITS.MELLISA MENTIONED THAT KOLWALSKY CONST. WAS A PHOENIX BASED COMPANY WITH AN OFFICE IN TUCSON.
