Optimus Engineering DATE: September 30, 2015 SUBJECT: PDSD Engineering and Floodplain Review REVIEWER: Elizabeth Leibold, P.E., CPM, CFM ACTIVITIES: T14OT00396, T15OT00065 (outlet) SUMMARY: PDSD Engineering has reviewed the As-built documentation for the drainage improvement to the existing culvert system along the Alamo Wash near Broadway -(Drainage report - Alamo Wash: Indian House Road to Wilmot Road CLOMR TDN and associated addenda, was prepared by Jiankang Wang P.E., CMG Drainage Inc, sealed 9/30/2014, and addenda including last addendum dated August 17, 2015 from Clint Glass P.E., CMG Drainage Inc., and associated as-builts for Alamo Wash Floodway Mitigation plans prepared by Jeff Behrana P.E., Optimus Civil Design Group, sealed 2/27/15). Inspection of the improvements by PDSD and TDOT occurred August 11, 2015. As-built documentation was submitted including as-built plans for inlet and outlet structure improvements, easement recordation information, field testing reports, certification by engineer of record, and substantial completion letter by civil engineer of record. PDSD Engineering accepts the as-built package for T14OT00396 and T15OT00065. Keep in mind that upon FEMA effective date of LOMR, subsequent permits within the revised area may be allowed to be issued based on the new LOMR data.
