5. Select Street Name from the list. You can type the first two letters or number to get closer to your choice in the list. Then select the rest of the address in the boxes provided.

6. Description of Work - Enter in Floodplain Use Permit for…and then put a description of the work being done or reference the building permit number associated with this floodplain use permit.

7. Complete the Applicant Info. (Note APA# is for applicants with an advance payment account. Leave blank is you do not have an APA account.

8. Complete the Contractor, Owner and Architect/Engineer sections as applicable.

9. Supporting Documentation Upload - Provide a complete cost estimate for the project unless it is for a new build/separate structure. You may also provide additional documents you think will be helpful.

10. Sign the application and submit.

12/09/2021 ERIC NEWCOMB BUILDING-COMMERCIAL REVIEW Reqs Change 1. Sheet G1 (General Building Summary); Sheet A1 (Floor Plan): This is a restaurant (A-2) that can be classified as a Group B Occupancy Classification per the IBC Section 303.1.1. Please indicate this on Sheet G1.
2. Sheet G1; Code Check: The Occupant Load uses an occupancy load factor of 100 SF/occupant on this sheet. Per the IBC Table 1004.5, the Occupant Load should be calculated in reference to the function of the space. Although this has a B Occupancy Classification, the Occupant Load should be calculated as 15 SF/occupant in the dining area (or use an actual seat count), and 200 SF/occupant in the Kitchen area. Please revise.
3. Sheet A1; Floor Plan: Please provide on this sheet the calculation for the required number of accessible seats and tables per the IBC Section 1109.11. Please indicate those seats and tables on the plan.
4. Sheet A1; Floor Plan: Per the IBC Section 2902.2(2), tenant spaces with 15 or fewer occupants require only a single unisex accessible restroom. This plan indicates a single restroom for 26 occupants. Please verify.

12/16/2021 ROBERT SHERRY MECHANICAL-COMMERCIAL REVIEW Reqs Change Provide justification for the ventilation rate used for the back of the house and for the prep area. They appear to be ventilated as if they are retail storage rooms but based on the food service drawing, FS-1, a significant amount of activity will be taking place in those two areas. The minimum required ventilation for an occupiable space is based on the occupant density and the activities taking place in the space. The ventilation rate is to be based on Table 403.3.1.1, IMC 2018, or an approved engineering analysis of the specific occupancy type. Reference: Section 403.3.1.1, IMC 2018.
