03/20/2008 ELIZABETH EBERBACH ENGINEERING REVIEW Denied TO: Kent Delph, PE, Grenier Engineering
SUBJECT: Country Inn Hotel Grading Plan Engineering Review
LOCATION: 665 N Freeway
ACTIVITY NUMBER: T08BU00131

SUMMARY: Engineering has received and reviewed the Site Plan package for review of the New MOB Building. The plan is not approved at this time. Address the following comments:

GRADING PLAN COMMENTS:
1) DS Sec.2-02.2.1.15: Address the following drainage report comments:
a) Show the limits of the 100-year floodplain and water surface elevation adjacent to project for the Santa Cruz River to assure that the FFE meets the minimum elevation. Label FEMA FIRM panel information.
b) Provide scupper calculations that demonstrate that the 10-year flood flow is contained under the sidewalk.
2) Please be aware that after review and approval, when you pick up the Grading Permit you will need a Grading Permit bond based on square footage of disturbance. The reseed bond amount is based on $0.05 per sqft disturbance. This bond must be in the City of Tucson's accepted format. It may be prudent to wait until the grading limits are clarified and the proposed disturbance area is accepted before attaining the bond. Additional items that the City of Tucson DSD may require include: a copy of ADEQ permit, grading permit fees (determined at the counter), and contractor information.
3) On sheet 1, address the following comments:
a) Provide square footage area of disturbance.
b) Remove signature line and type 2 grading notation as the City of Tucson DSD Engineering approves with a stamp.
c) Label grading plan activity number T08BU00131 on the plans.
d) Revise note 14 to state City of Tucson not Pima County.
e) Add to grading note 11 that any slopes steeper than 3:1(H:V) may need to be reconstructed or will be required to have rip rap with filter fabric.
f) Add a grading note, or show and label areas on plan view, describing whether revegetation or type of other permanent cover materials will be required to be placed for all remaining disturbed areas prior to final grading approval.
4) On sheet 2 address the following comments:
a) DS Sec.2-02.2.1.A.12: Modify truncated domes on detail 3/2, so that they are located along the flat transition of pedestrian and vehicle circulation area.
b) For detail 11/2, address the following:
i) Provide call-out D, or clarify outlet erosion protection.
ii) Keynote C references a detail above - provide clarification or detail.
iii) Provide note or clarify wall opening spacing. Otherwise clarify whether outlet weirs are needed for wrought iron fence, or provide elevations to show concentrated flows on planview.
5) DS Sec.2-02.2.1.19: Dimension right-of-way for I-10 Freeway.
6) DS Sec.2-02.2.1.20: Show all easements on the plan with docket and page. On utility sheet, clarify the individual utility lines. If there are any fiber optic lines must be identified and notated.
7) On sheets 5 and 6, delineate and clarify the following:
a) Project boundary.
b) Grading limits.
c) Any additional offsite construction / maintenance easements.
8) DS Sec.3-01.5.1.A.1: Show sight visibility triangles for proposed and existing conditions on plan view. Provide note: Fill, fences, walls, vegetation cannot be located within the sight visibility triangle between 30" and 72". Other than trees as noted below, plant materials located within existing and future sight visibility triangles shall be limited to ground cover or low-growing vegetation of a species that will not grow higher than thirty inches. Trees may be located within existing and future sight visibility triangles only if clear of leaves and branches to a height of at least six feet above grade upon installation, and at all times thereafter. The trunk caliper of any species selected may not exceed twelve inches in diameter at maturity. Trees with multiple trunks are not allowed. Trees may not be planted in a line that could result in a solid wall effect as viewed from the motorist's perspective. The number and location of trees within existing and future sight visibility triangles may be restricted or modified by the City of Tucson in order to preserve visibility.
9) The Stormwater Pollution Prevention Plan (SWPPP) does not meet the minimum requirements of the AzPDES Construction General Permit (CGP). Revise the SWPPP according to these comments:
a) Assure that the new NOI form is is used as well as signed and completed, for your project per recent ADEQ update to the CGP. The updated NOI, NOT, and Waiver Certification forms can be found at the ADEQ websites at: http://www.azdeq.gov/function/forms/appswater.html#cgp http://www.azdeq.gov/environ/water/permits/download/constnoi.pdf
b) Label receiving waters on a plan view map in report or on exhibit. On exhibit if a map is not provided labeling receiving waters. Provide a general note on the SWPP exhibit.
c) Provide registrant seals for SWPP exhibit per COT Stormwater Ordinance.
d) On SWPPP exhibit sheet address the following comments:
i) Revise the construction entrance to begin at edge of impervious surface such as pavement or concrete at edge of property for a minimum of 50-ft.
ii) Clarify property boundary and grading limits; delineate both on the SWPP exhibit.
iii) Designated concrete washout locations are required; show location of concrete wash-out area on plan view of SWPP exhibit. Delineate limits of designated concrete washout area such that it is not located in or near the basin or water harvest areas.
iv) It is typical, but not required, to place installation details on a SWPPP exhibit sheet. Consider placing the particular control measure installation details in a SWPP exhibit. Consider providing a silt fence detail that will be useable to the contractor; provide a detail without drain rock along the base of the silt fence.
e) For inspection purposes, please fill out the enclosed "AZPDES - Posting Requirements" green sheet, and post at construction entrance of the site at beginning of construction activities and maintain this posted document throughout project construction.
10) Provide geotechnical recommendations for setback from ponding areas such as water harvest basins or other drainage structures that may have ponded water.
11) Provide depressed curbing to allow for onsite drainage to enter into identified water harvesting areas. Verify that water harvesting areas do not affect proposed sidewalk or provide scuppers.
12) Call out all curb radius dimensions on a grading planview sheet.
13) A Floodplain Permit application will be required for this project. Floodplain Permit application fee and review is $150 and $50.
14) Resubmittal is required. Submit the resubmittal package to the permit clerks under T08BU00131 including:
a) 3 copies of the revised grading plan.
b) 3 copies of the revised SWPPP
c) 1 copy of Geotechnical report and any addenda.
d) 1 copy of revised drainage report.
e) Floodplain Permit application and fee.
f) Response letter.

Address all the comments for resubmittal. If there are any questions or would like to set up a meeting to go over comments, you may contact me at 837-4934.
