| 12/16/2004 | Blanca Espino | ENGINEERING | REVIEW | Denied | DATE: December 27, 2004 ACTIVITY NUMBER: T04BU02919 PROJECT NAME: Family Dollar PROJECT ADDRESS: 3960 North Stone Ave. PROJECT REVIEWER: Patricia Gilbert, Engineering Associate The following items must be revised or added to the Storm Water Pollution Prevention Plan (SWPPP). Please include a response letter with the next submittal that states how all comments have been addressed. RESUBMITTAL REQUIRED: STORM WATER POLLUTION PREVENTION PLAN. 1. Each operator is responsible for submitting a completed NOI to ADEQ and to the City of Tucson. The contractor for the development is required to submit an NOI to ADEQ and a copy to the City of Tucson. Submit a copy of the completed and signed NOI. (Part IV.F) 2. In the narrative include a copy of the AzPDES permit provided by ADEQ. (Part IV.F) 3. The certification statement needs to be signed prior to approval of the SWPPP. This assures the City of Tucson prior to issuance of a grading permit that there is a designated responsible party for the SWPPP. 4. The intended sequence of disturbance needs to include all aspects of the proposed development. This includes utility installation, paving, constructing the building, landscaping, etc. Revise the intended sequence of disturbance to be specific to all phases of the development. Part IV.C.2.b. 5. It is recommended that a separate SWPPP site map is included with the narrative rather than using the site/grading plan for the SWPPP. The SWPPP site map should be labeled as such, "SWPPP Site Map." This is particularly useful once the development of the site has started and field inspections have started. When a SWPPP site map is included on the same page as the grading and site plan, the SWPPP information is difficult to decipher between the required grading and site plan information. Please review Part IV.C.3. for the required information on a SWPPP site map. Remove zoning information and any information that is not a requirement of Part IV.C.3. 6. Identify locations of where temporary stabilization occur. Identify the location of the silt fencing that is described in Section B of the narrative. Part IV.C.3. 7. On the location map show and identify receiving waters within 1 mile of the site. Include this information in the narrative. (Part IV.C.2.e) 8. It is noted that the storm drain on Stone Ave has been identified on the site/grading map. However this information must be on the SWPPP site map. Revise. Part IV.C.3.g. 9. Provide the location of the stabilized construction entrance and a detatil on the SWPPP site map. (Part IV.D.2.c, Part IV.D.8.a.) 10. It is recommended that details are used for all proposed SWPPP Controls this ensures proper installation. For any proposed SWPPP controls add a standard detail to the SWPPP site map. Part IV.D.2b. 11. Paragraph 3, Section B, states that litter, construction debris and chemicals will be prevented from entering Stone Ave or Roger Rd. Explain how this will be accomplished. Be specific. Part IV D.2.c. and Part IV D.3. 12. The first paragraph in section C states that existing vegetation will be preserved in the designated landscape areas. Typically within landscape areas is new required vegetation. Is all the vegetation within the landscaped areas preserved or is it new vegetation? If the vegetation is existing and is proposed to be preserved indicate the location of the preserved (undisturbed) vegetation. If it is new vegetation/landscaping indicate on the SWPPP site map. 13. The first paragraph in section C states that all disturbed areas shall be covered immediately with native material. The entire site is going to be disturbed. The site plan indicates asphalt, concrete, decomposed granite, etc to stabilize the project site. The SWPPP narrative must be site specific. Please revise the narrative to reflect the requirements of Part IV.D.4.a. 14. Provide an actual form/checklist that includes the following information. Please review handouts for ideas on inspection and records of stabilization format. The following records of stabilization must be maintained; Dates when major grading activities occur. (Part IV.D.4.c.i) Dates when construction temporarily or permanently ceases on a portion of the site. (Part IV.D.4.c.ii) Dates when stabilization measures are initiated and completed. (Part IV.D.4.c.iii) 15. Section E, paragraph 1, states, "Due to the site's aridness, the site inspections will be conducted at least once a month and before and after significant rainfall event." Please revise to read "Due to the site's aridness, the site inspections will be conducted at least once a month and before rainfall is predicted and within 24 hours of the end of storm/rainfall event of 0.5 inches." Part IV.H.2. 16. Include three copies of the SWPPP and the grading plan with the next submittal. |
