RESUBMITTAL REQUIRED: Grading Plan, Drainage Report and SWPPP

The Grading Plan (GP) and Drainage Report (DR) cannot be approved as submitted. Please address the following review comments prior to the next submittal.

Grading Plan:

1. Please include a response letter to the comments along with the corrected copies of the GP.
2. The approved Dev. Plan is required to be submitted with the GP.
3. Show all symbols in the legend and/or edit as required.
4. Sidewalks must connect to the circulation path located in any adjacent street per D.S 2-08.4.1.D. The s/w in the SW corner of the PAAL at the end of the north parking spaces shall be extended to the s/w on Tanque Verde. The sidewalk at the end of the North parking space shall have a minimum of a "turnaround" at the end of the sidewalk for a person in a wheelchair that may decide to use that sidewalk, will be required per ANSI standard A117.1, 2003 edition.
5. Correct section call outs to read A/C1.1 and B/C1.1.
6. Add note: "Depress all landscaped areas a maximum of 6" for water harvesting"
7. Edit the basin(s) maintenance responsibility note to read per S.M.D.D.F.M. 2.3.1.6 C 1 and 2.
8. List Development number on plans (D04-0027).
9. List all Cut and Fill quantities in cubic yards.
10. Add note to general notes:"All Grading shall conform to D.S. 11-0.0.
11. Show Phase numbers and delineate. Each phase must function independent of future phases.
12. A permit or a private improvement agreement will be necessary for any work performed within the Right-of-way. Contact Permits and Codes at (520) 791-5100 for permit information.
13. Call out peak discharges entering and exiting the site.
14. Detention/Retention:
· Show detail of rip-rap splash pad.
· Dimension basin and provide spot elevations.
· Peak discharge at basin's outlet.
15. Basis of elevation shall be a COT Bench Mark based on City of Tucson Datum, including City Field Book and page number per D.S. 2-02.2.1.23 and match the Development plan.

Drainage report:
1. Please include a response letter to the comments along with the corrected copies of the DR.
2. Provide a detail showing dimensions for the Ret./Det. basin.

Inspections
5. Provide example forms for Compliance Evaluation Reports (Inspection Reports). At a minimum the forms must include:

b. Name(s) and title(s) and qualifications of person(s) making the inspection. Qualifications must either be on/attached to the report or alternatively, if the SWPPP documents the qualifications of the inspectors by name, that portion of the SWPPP may be referenced. (Part IV.H.5.b)

Weather information for the period since the last inspection (or since commencement of construction activity if the first inspection), including best estimate of the beginning of each storm event, duration of each event, time elapsed since last storm event, and approximate amount of rainfall for each event (in inches) . (Part IV.H.5.c)
d. Location(s) of discharges of sediment or other pollutants from the site. (Part IV.H.5.d)
e. Location(s) and identification of BMPs that need to be maintained; failed to operate as designed or prove inadequate. (Part IV.H.5.e)
f. Location(s) where additional BMPs that do not exist at the time of inspection are needed. ((Part IV.H.5.f)
g. Corrective actions required, including any changes to SWPPP necessary and implementation dates. (Part IV.H.5.g)
h. Identification of all sources of non-storm water and the associated pollution prevention control measures. (Part IV.H.5.h)
i. Identification of material storage areas and, evidence of or potential for, pollutant discharge from such areas. (Part IV.H.5.i)
j. Compliance certification statement. (Part IV.H.6)
k. Signature line with certification statement. (Part IV.H.6)
l. Signature Requirements
6. Include a certification and signature for each operator in accordance with Part VII.K. (Part IV.J.1)
7. All NOIs must be signed and certified as follows:
a) For a corporation: By a responsible corporate officer. For the purpose of this Part, a responsible corporate officer means:
i) A president, secretary, treasurer, or vice-president of the corporation in charge of a principal business function, or any other person who performs similar policy or decision-making functions for the corporation; or
ii) The manager of one or more manufacturing, production, or operating facilities, provided, the manager is authorized to make management decisions which govern the operation of the regulated facility including having the explicit or implicit duty of making major capital investment recommendations, and initiating and directing other comprehensive measures to assure long term environmental compliance with environmental laws and regulations; the manager can ensure that the necessary systems are established or actions taken to gather complete and accurate information for permit application requirements; and where authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures.
b) For a partnership or sole proprietorship: By a general partner or the proprietor, respectively; or
c) For a municipality, state, federal, or other public agency: By either a principal executive officer or ranking elected official. For purposes of this section, a principal executive officer of a Federal agency is the chief executive officer having responsibility for the overall operations of a principal geographic unit of the agency (e.g. Regional Administrators of EPA.).
8. Reports and Other Information: All NOTs, SWPPPs, reports, certifications, or information required by this general permit and other information requested by the ADEQ or authorized representative of the ADEQ shall be signed by a person described in Part VI.K.1 or by a duly authorized representative of that person. A person is a duly authorized representative only if:
d) The authorization is made in writing by a person described in Part VII.K.1;
e) The authorization specifies either an individual or a position having responsibility for the overall operation of the regulated facility or activity, such as the position of manager, operator, superintendent, or position of equivalent responsibility or an individual or position having overall responsibility for environmental matters for the operator. (A "duly authorized representative" may be either a named individual or any individual occupying a named position.); and
f) The signed and dated written authorization is included in the SWPPP and submitted to the ADEQ upon request.
9. Certification. Any person signing a documents under the terms of this permit shall make the following certification: I certify under penalty of law, that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gathered and evaluated the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing violations.
10. Under the RETENTION OF RECORDS, an associated bound narrative is supposed to be included with this submittal but was not. Include with resubmittal.
