The following items must be revised or added to the Storm Water Pollution Prevention Plan (SWPPP). Please include a response letter with the next submittal that states how all comments have been addressed.

RESUBMITTAL REQUIRED: STORM WATER POLLUTION PREVENTION PLAN.

1. There appears to be a discrepancy between the SWPPP narrative and the NOI. The section on the NOI where non-stormwater discharge from the construction activities is listed it states that there will be "None." However in the SWPPP Narrative section XI, there are six non-stormwater discharges listed. Revise the NOI to include what is listed in the SWPPP narrative.

2. Identify off-site material storage areas used solely by the project. If there are none please state so in the narrative and on the site map. (Part IV.C.6)

3. Document areas where existing vegetation will be preserved. If there are none please state so in the narrative and on the site map. (Part IV.D.4.a)

4. In areas where construction has temporarily or permanently ceased, stabilization measures must be implemented within 14 days. If construction will resume within 14 days, then no stabilization measures are needed. Explain how the site will address stabilization if construction temporary or permanently ceased. (Part IV.D.4.b.)

5. In the narrative, section X, Inspection Schedule and Maintenance of Controls, it states sediment will be removed when it reaches a third of the height of the silt fence. This is incorrect. When design capacity has been reduced by 50% from accumulated sediment, the sediment must be removed from the stormwater control. Revise the SWPPP narrative and add a general note to the site map. (Part IV.E.3)

6. Show existing drainage patterns. (Part IV.C.3.a)

7. On the site map show the locations of off-site (if there are any) and on-site material, waste, borrow areas, equipment storage and wash out areas. (Part IV.C.3.e)

8. Include this statement in an appropriate section of the SWPPP Narrative, "Report oil or hazardous materials releases in excess of reportable quantities, or that may endanger human health or the environment, within 24 hours to ADEQ: (602) 771-4466 FAX (602) 771-4505." (Part VII.L.5.a)
