11/02/2016 TDOT-Routine Barricade Inspect Continued Magnum backfilling trench with type 2 CLSM. at app. 0345 a vehicle drove in to the closure and across the trench. Driver did not stop and abandoned the vehicle in a private parking lot North of the job site. At the time of the incident all traffic control devices were set per the approved traffic control plan and were in conformance with accepted standards. A more detailed description of the incident along with pictures of the closure were sent to David Stiffey via e-mail.
10/18/2016 TDOT-Routine Barricade Inspect Continued SWG was on site at 9pm to inspect and wrap 4" steel line over new H2O. Magnum excavated and located unknown 2'' conduit, and encased COT Electric conduits.
10/12/2016 TDOT-Routine Barricade Inspect Continued Magnum ran into issue with plating configuration. Not enough AC was milled to allow for plating of trench for new line due to plan change. Will pull off Campbell Road closure ASAP and continue working on 2" service on Allen Rd.
10/05/2016 TDOT-Routine Barricade Inspect Continued Magnum excavated up to water line overnight. Water line has a thrust block connection that is too close to area where tap was to be installed. Hensel Phelps took pictures and sent information to EOR for the project to design alternatives and submit to Tucson Water. Plates were in place and acceptable and TC was removed at app 0430.
10/05/2016 TDOT-Routine Barricade Inspect Continued Trafficade didn't arrive until 8:35pm. Scheduled to set up at 8pm. SWG is onsite to pothole for 4" Steel. CSC is on site to sawcut and mill for new trench/plates.
10/04/2016 TDOT-Routine Barricade Inspect Continued Magnum worked overnight to sawcut, prep for plates and locate utilities. Inspected traffic control setup and plate installation and found them to be acceptable.
10/04/2016 TDOT-Routine Barricade Inspect Continued Arrived at 7:50pm. Trafficade began setup at 8pm . Existing H2O line conditions will not allow plan as-is. Will need plan revision by TW. Will need additional asphalt removal.
10/03/2016 TDOT-Routine Barricade Inspect Continued Arrived at 7:50pm. Trafficade had already set up TC. Permit does not allow set-up until 8pm. Warned PM from Hansel Phelps that permit times must be adhered to. Trafficade scheduled to begin tear down at 4am. Milling for plates began at 8:30pm

09/29/2016 TDOT-Routine Barricade Inspect Continued Meeting with TDOT Electric shop, Tucson Water, Hansel Phelps, and Magnum. Discussed night time work starting 10/3. Trenching into Campbell Ave. T-Top trench. CLSM #2 backfill.
09/14/2016 TDOT-Routine Barricade Inspect Continued Woking on expired permit. Reminded of potential fines for doing so. Flaggers had no paddles. Told to get Stop/Slow paddles. Stop work and pull off until new permit issued.
