Date Description Inspector Results Comments
11/02/2009 INSP - NIGHT Approved COSTUME PARTY, HAUNTED ROOM AND FLAME EFFECTS PERFORMANCE 10/31/2009

The haunted room was approved prior to it's use by the public. Only about 40 occupants in the building at the time of my inspection. No violations noted.

The flame effect performance that had been scheduled for the night was not approved through an application for permit with the Tucson Fire Department. The performer was unaware of the required permit and approval process to handle flame effects within the City of Tucson. I allowed the performance to continue once all the safety precautions, Fire Code requirements and audience set backs were in place. The performance lasted about an hour with two fifteen minute breaks. The lead performer is scheduled to take the Flame Effects Responsible Party test within the next month.
