The flame spread rating for this material has not been supplied to the Tucson Fire Department as requested on 2 occasions. This material is in all 72 rooms. (Photo)

4). Failure to remove unauthorized locks from doors to practice rooms. -TFC 1008.1.8.4
Flush bolts are being used on the entrance side of each door. In addition, flush bolts are padlocked, on the entrance side. There are 2 of these locks on each of the 72 doors. (Photo)

5) Failure to provide key box for access. -506.1
A key box is required because access in this building would be extremely difficult due to metal door will keypad access, and no other immediate access to stairwell. Manager of building Mr. Burnham, has been notified of the process to obtain key box.

6). Failure to comply with Tucson Fire Department written order to obtain a Certificate of Occupancy. TFC 109.2.2
On 2 occasions, a written order, by certified mail, was given to Francisco Studios to obtain a Certificate of Occupancy. Application was mailed to Francisco Studios with the original report. Non-compliance as of this date.
