b) A page indicating the scope of the modifications pursued, and the reason(s) for the proposed modifications;
c) Letter from property owner authorizing the modification request;
d) Site plan included in the approved design package, and new proposed site plan, if applicable;
e) First floor-floor plan included in the approved design package, and new proposed first floor-floor plan;
f) Color elevations included in the approved design package, and new proposed color elevations;
g) Color and materials included in the approved design package, with new proposed color and materials, if applicable; and
h) Any other proposed modifications to the approved design package;
i) Once I receive the information above, additional information may be required, and payment of design/historic review fees will be requested.

Based on the proposed revision on the approved design package, it will be determined if a major or minor review process will need to be followed. Preliminarily, it seems that a minor review process will be needed, as determined by the Design Professional and the Historic Preservation Officer on 3/7/2022.
