Per Code Enforcement Inspections report, code violations described for work performed without permits at this property. The electrical scope of work included but is not limited to: Replacing, relocating or installing new Receptacles (possibly branch circuit wiring and lighting, new circuit breakers) for the Kitchen Remodel. It was also noted in the inspection report that some of the Sub- Panels were missing hardware, covers, and there was paint over-spray on the internal parts of the panels as well as circuit breakers. Electrical equipment not identified, marked, and other code violations concerning the wiring methods of equipment and or appliances such as the water heaters.

This is a commercial project, requiring all documents to be prepared, sealed, signed, and dated by a registered design professional. Provide complete electrical plans sealed by a Registered Electrical Engineer registered in the State of Arizona. Reference: Arizona Revised Statutes 32-125, Board of Technical Registration Rules R4-30-304.

Include an electrical site plan including any exterior power and lighting modifications if applicable.

Please see instructions below to apply for a Floodplain Use Permit. A separate Floodplain Use Permit will be required for each Building (BLDG 1-6) for this project.

Instructions:
1. Is this for a new or existing activity application? - Please select New Application.

2. Submission Type - Select Floodplain Use Permit.

3. Project Name - Enter a name for your project.

4. Type of Permit - Select Commercial or Residential (Residential is for work on single family residences and duplexes.)

5. Select Street Name from the list. You can type the first two letters or number to get closer to your choice in the list. Then select the rest of the address in the boxes provided.

6. Description of Work - Enter in Floodplain Use Permit for…and then put a description of the work being done or reference the building permit number associated with this floodplain use permit. Also, make reference to which building this permit is going to cover.

7. Complete the Applicant Info. (Note APA# is for applicants with an advance payment account. Leave blank is you do not have an APA account.

8. Complete the Contractor, Owner and Architect/Engineer sections as applicable.

9. Supporting Documentation Upload - Provide a complete cost estimate for the project broken out per building.

10. Sign the application and submit.
