The following changes/redlines are required for approval: Sheet GN02 Rewrite note 7 to indicate that all other sign panels are ASTM type IV (switching this sentence to the end of the note: "All warning sign panels shall be fluorescent yellow ASTM Type XI sheeting. All school zone signs shall be fluorescent yellow-green AS P101 Need to see full west leg to check signing/striping P101 Instead of a Type 2 object markers, use Yellow reflective PVC post P101 Use advance lane control signs on the median that shows dual left turns at the beginning of the turn bay and then again on median nose. This sign should include the word message "No U-Turn" underneath at both locations. P101 The lane control sign on the right curb is an advance lane control sign. Install sign at approximately the beginning of the left turn bay. It's OK to use it a second time where it is shown, but it is unnecessary because of the advance lane control signs P101 Lots of guide marks in this intersection. OK to delete the guide marks for the WB Elm left turn. The phasing will be such that this turn occurs by itself. P101 Additional signs are shown on the signal sheet TSO1 This detail does not show the cradle. It is OK to point to the schematic of the cradle on the horizontal sign support (picture of the signal), but only if the sign installation is included in the Special Provisions. TS02 Mount on the A-pole on the NEC a 30x36 "LEFT TURN YIELD ON FLASHING YELLOW ARROW" sign. TS02 Mount on the mast arm next to the left-turn FYA a sign a 42x48 LEFT TURN YIELD ON FLASHING YELLOW ARROW sign. Move lane control sign to pole if necessary. TS02 Block numbers are either 1900 E or 1700 N for street name signs. TS03 Use E Elm ST 1700 N for both Elm Street signs TS03 Use N Campbell Ave 1900 E for both Campbell Ave signs TS03 Note that street name signs are double sided TS03 Add note: Ths sign panel shall be straight and true with no bowing prior to or after applying the sheeting material. 06/26/2017 | RICHARD LEIGH | DOT ENGINEERING | REVIEW | Reqs Change | June 23, 2017 SUBJECT: Banner University Medical Center Tucson BUMCT Off-Site Expansion & Renovation PROJECT REVIEWER: Richard Leigh, TDOT Engineering Review RESUBMITTAL REQUIRED: TDOT Permit Review Section has reviewed the subject PIA paving plan and does not recommend approval at this time. The following comments need to be addressed and plans revised prior to final approval. Please include a response letter addressing the following comments and return any redlined drawings with subsequent submittal. 1. Sheet 1: Include the plan number "U-2017-007" in the title block of each sheet. 2. Sheet 1: Include the PIA activity number "T17PI00030" on cover sheet. 3. Use City of Tucson Department of Transportation Drafting Standards pursuant to Active Practice Guidelines 8-1552-002 for plan layout and title blocks. The drafting standards and Private Improvement (PIA) guidelines can be found at https://www.tucsonaz.gov/tdot/private-improvement-agreement. 4. Sheet 1, Revise Construction General Note 2 to indicate the use of City of Tucson, TDOT Mix#2. asphaltic concrete. This is a new mix that is different from what is called out in section 406 of the PAG Standard Specifications. 5. Sheet 2: Include the following general construction notes: All changes to these plans must be cleared by the City of Tucson City Engineer's Permits and Codes Section, prior to construction. The Engineer of Record shall submit to the City for review any changes to the approved plans prior to construction. Additionally, the Engineer of Record shall certify all changes meeting all applicable standards, codes, and ordinances. An excavation permit shall be obtained from the City of Tucson Transportation Department prior to any work within the Public right-of-way. A pre-construction field meeting shall be conducted with the City of Tucson Transportation inspector prior to any work within the public right-of-way. ‰ The extent and type of surface treatments/repairs indicated on the plans may not be all inclusive of the work that is needed. Additional repairs, paving, or replacement may be needed depending on field conditions. The contractor shall contact and/or coordinate any work with COT Streets & Traffic Maintenance Division, Median Island Maintenance Section (791-5279, ext. 234) prior to disturbing any COT/DOT landscaping or irrigation systems. Additionally, the contractor shall repair or replace any disturbed COT/DOT landscapingor irrigation systems, including replacement of any damaged plants. Contact Gary Wittwer @ 400-2645 for inspection and permits for landscaping. For irrigation removal/replacement contact Paul Rosenboom at 237-7386. The PIA Inspector shall verify that the "Saw-cut" line is located within competent pavement. Adjustments to the plan shall be documented and As-built; a one foot minimum shall be provided. The contractor shall contact and/or coordinate any work with COT Streets & Traffic Maintenance Division, Electric Shop 791-3154 prior to relocation or placement of any street lighting or traffic facilities. Contact Ernie Encinas at 791-3154 for street lighting/traffic signal plan review. The contractor shall comply with all applicable Occupational Safety and Health Administration regulations 6. Sheet 4: Label new right-of-way limits with recordation information. 7. Sheet 5: Indicate the placement of vertical curb at back of curb access ramps. 8. Sheet 8: Provide a vertical profile for the proposed new curb lines of the deceleration and left turn lanes. 9. Please Refer to redline drawings for additional comments and comment details.
