PROJECT REVIEWER: Richard Leigh, TDOT, TDOT Eng. Review

RESUBMITTAL REQUIRED: TDOT Permit Review Section has reviewed the subject PIA paving plan and does not recommend approval at this time. The following comments need to be addressed and plans revised prior to resubmittal. Please include a response letter addressing the following comments and return redlined drawings with subsequent submittal.

1. Include the plan number "U-2016-007" in the title block of each sheet.

2. Include the PIA activity number "T16PI00005 on cover sheet.

3. Sheet 5-7: Provide a profile view of roadway TDOT Office procedure No. 8-1552-002 Types of Sheets 2.E.

4. Sheet 2: Remove Detail 1, detail is not applicable.

5. Sheet 2: Revise detail 2 to indicate percent of grade of landing and ramp section. Additionally, provide details of the ramp curb.

6. Sheet 5: Revise drawing to eliminate proposed driveway entrance at +/- 11+00 Rt Allen Rd. Sewer manhole can be accessed from roadway.

7. Sheet 5-7: Revise drawing to depict a flared side on both sides of the curb access ramps per PC/COT Std. Dtl. 207 Sheet 4A.

8. Sheet 5: Label newly dedicated right-of-way along the north side of Allen Road with recordation information.

9. Sheet 5: Revise drawing to indicate the finished surface treatment details for the proposed median island.

Traffic Signal Note 16 should read "… law enforcement officer SHALL PROVIDE POINT CONTROL DURING THE CHANGE-OVER TO THE NEW SIGNALS AT THE CONTRACTOR'S EXPENSE."

Conductor Note 4 should read "IMSA 16, 7, and 4 conductor cables…"

Identify existing poles D through J

Show existing video detection camera for eastbound traffic on luminaire mast arm of pole "D".

Install "Q" head on SEC "A" pole for SB phase 5 turns.

NEC - intercept existing conduits and extend to new #7 pull box with extension.

Include note that intersection shall require new 16 conductor IMSA to be run, un-spiced, from the controller cabinet to each pole's terminal block.
