02/28/2014 ELIZABETH LEIBOLD ENGINEERING REVIEW Denied TO: Ryan Stucki P.E, EEC Engineering
SUBJECT: St Mary's Onsite Parking Lot Site/Grading Plan Engineering 1st submittal Review
LOCATION: 395 N Silverbell Rd
ACTIVITY NUMBERS: T14OT00163, DP14-0016

SUMMARY: Engineering has reviewed the package. Site plan is under DP14-0006. Previous temporary use permitting has occurred for this parking area; this project proposes final grading and pacing improvements for parking. Engineering recommends approval and issuance of grading permit once the following Technical Memorandum comments are addressed. The conditions will apply to the grading permit DP14-0016.

DRAINAGE COMMENT:
Please revise the Technical Memorandum letter to address the following comments:
1) Revise the statement to include the reference to previous levee report from EEC which refers to a flow that enters the area that is significantly larger than the watershed indicated in this memorandum. Call to discuss or meet to go over report.
2) Add reference to minimum RFE for generator (as shown on grading sheet 2.).
3) Revise the statement to include the title "Drainage Statement". Also add permit activity numbers to plan.

GRADING PERMIT CONDITIONS:
The following 8 conditions apply to permit DP14-0016:
1) Call for Pre-construction meeting, intermediate and final inspections.
2) This plan acts as the Grading Plan for reconstructed parking lot in the center of the hospital campus.
3) Temporary erosion/stormwater controls may be used at entrance to existing pipe and be field adjusted with discussions with Contractor and Engineer of record and Inspector as necessary.
4) There shall be no disturbance outside of the approved disturbance/grading limits.
5) Any revision to the Grading Plan may require a re-submittal of a revised grading plan for review. Contact PDSD Engineering to discuss changes in grading design.
6) If grading construction is expected to last longer than the expiration date of the grading permit, contact PDSD to renew/extend the Grading Permit. If Final Grading Inspection has not been completed before the Grading Permit expires, and the permit has not been renewed, additional fees and reviews may be required.
7) The permitee shall notify PDSD when the grading operation is ready for final grading inspection.
8) Prior to request for final grading inspection approval, all erosion control improvements shall be installed and accepted by PDSD including rock / established revegetation for all disturbed areas if not improved as per approved plan. As-builts shall be provided.
