The following items must be revised or added to the plan. Please include a response letter with the next submittal that states how all comments have been addressed.

1. See redlines on the submitted drawing Shts 1 & 2 and address the additional redline comments.
- Please fix or include omitted signing notes on plans Sht 1.
- Please do replace with new; post and D11-1 sign on Sht 2
- Please do use TDOT sign post & foundation Standard 8-2A

10/21/2013 LANCE PETERSON DOT STREET MAINTENANCE REVIEW Denied We have reviewed this plan and have the following comments:

1. Sheet 1 - Please provide a Streets and Traffic Maintenance Signature line in the title block.
2. Sheet 1 - Note 5 - This note mentions matching pavement section of 22nd Street. How was this determined given that the project is off of 18th Street and Main Avenue. What is the pavement section?
3. Sheet 2 - The two driveways off of 18th should continue through the new sidewalk or need to call for a 6" thick concrete sidewalk at these locations.
4. Sheet 2 - The driveway centered at Station 10+69.01 appears to need an additional station and offset for the start of the transition or the start of the depressed curb to locate.
5. Sheet 2 - Is there another curb transition needed for the driveway centered at Station 11+22.75?
