First, you have used the Town of Pima as the basis for the calculations; the town is located in Graham County and has a different climate than the City of Tucson. You can find Pima County<4,000 feet in the drop-down menu under "Code" on COMcheck. Using your input values, COMcheck shows that the addition fails by 2% in Pima County. Second, in this climate zone, the slab does not require any insulation (you can ignore the slab if you wish) but the input value for the slab is the slab perimeter, not the slab area.
