This plan was designed under the 2006 UPC; an additional cleanout is required for "each aggregate horizontal change of direction exceeding one-hundred and thirty-five (135) degrees". Do not conflate the two plumbing codes, the 100 feet mentioned in item #5 is strictly in the 2006 IPC. [Third comment: Comment not addressed.] [Second comment: The cover sheet for this plan set indicates that the 2006 Uniform Plumbing Code (UPC) has been used as the basis for design. Note #5 is appropriate for the 2006 International Plumbing Code (IPC) but is incorrect under the UPC. This has a direct impact on the required cleanouts for the restroom. As currently drawn, the restroom cleanouts are acceptable per Section 708.3.3, IPC 2006 but additional cleanouts will be required per Section 707.5, UPC 2006 (alternatively, the 4" drain could be straightened out to eliminate the problem).
