ACTIVITY NUMBERS: Floodplain Use Permit T12OT01004, Grading Permit T12BU00832, & D11-0032 SUMMARY: Engineering has reviewed the revised sheets and other project completion documentation submitted for final grading approval, and does not recommend final approvals of the as-built Final Revised Grading Plan and Floodplain Use Permit. Please address the remaining completion items. Permitted project scope included removal of old drainageway crossing, channel replacement at old crossing, installation of new bridge crossing, installation of drainageway access ramp, installation of stormdrain pipes, installation of RCP connection to new stormdrain pipe near outlet to existing drainageway, compaction of soils adjacent to public drainageway, constructing central basin, compaction of fill at northwest side of project adjacent to top of embankment of existing drainageway, private roadway, grading for site, and other improvements as shown on the approved plans. A bond was submitted for the excavation area not shown on the approved plans. After discussing the access/maintenance easements with TDOT, it appears that PSOMAS obtained 2 easements in August 2012 after the issuance of the grading permit (construction access was needed at the new crossing). Existing Bridge Removal acceptance is expected from TDOT from discussions with Richard Leigh once final inspections are completed. A few remaining items are still expected to be completed by Inspectors, including installation of remaining railing and erosion protection for new bridge crossing abutments. ENGINEERING COMMENTS FOR FINAL APPROVAL FOR PROJECT COMPLETION: Tech Man Sec.2-01.13: Please address the following comments: 1) For the as-built sheets, address the following comments: A) Revise as-built sheet(s) to include recordation data for ramp-related access & maintenance easement granted to City and the access & private maintenance easement across the new bridge. See the following link for easements' data: http://tdotmaps.transview.org/Images/RealEstate/INFO/Inf164.pdf 2) Regarding substantial completion letters, address the following: A) The test reports indicate that roadway base course did not meet specifications for compaction. Please confirm with soils engineer that project is substantial complete for the work within their area of responsibilities and/or is in accordance with the approved soils engineering report. B) Revise cover letter sheet or substantial completion statement to include reference to bond that was provided to complete the excavation pit-basin area. 3) While awaiting for Inspectors' confirmations regarding improvement completion, address the following: A) Assure all railing is provided per approved plans, as last inspection indicated the railing was still being installed. In particular, railing for approved basin at far east corner of project, railing for new crossing, railing along east side of phase 1, and any replacement railing along Brichta Wash that may have been removed for construction.
