Sheet 2 of 3

1. Note (3rd Bullet): add notation that Sylvania F96T12/CW/VHO or other VHO
lamp with a higher mean lumen rating than 9604 may not be installed as a
substitute on this billboard. Sylvania F96T12/CW/VHO/SS (9100 mean lumens)
is the preferred lamp for this 5-lamp (per face) installation.

2. Note: add notation that no junction boxes to be installed or used other
than those depicted on plan.

3. Note: add notation that number of #12 conductors specified includes
ground wire.

4. Partial Plan View: use this top view to provide a full schematic of all
junction boxes and connecting conduit; a note that there is a junction box
under each of the depicted ballasts will suffice for those junction boxes;
in particular, depict the junction box where the circuits are to branch for
the two different faces, including its specific location and the change in
number of conductors; alternatively, a three dimensional schematic showing
the junction box layout may be provided (including ballasts and the changes
in number of conductors).

5. Elevation: show incoming conduit connecting at a junction box below one
of the three ballasts rather than adding a fourth junction box; otherwise
indicate why the fourth box is needed.
