SUMMARY: After email correspondence with consultant, the revised Floodplain Use Permit and SWPPP Application plan submittal were re- reviewed again by Planning & Development Services Department Engineering Division. Meeting was held 19FEB10 to go over 1st submittal plans prior to this resubmittal. Additional correspondence with consultant occurred to discuss utility issues for the project. Subsequent electronic revisions were provided 29MAR10. The latest revised plan package was resubmitted and reviewed 8APR10. Planning & Development Services Department Engineering recommends conditional approval.

CONDITIONS OF APPROVAL:
1) e) On GD03, show any update to south entry area per discussions with TDOT. Provide mountable curb detail for emergency access area on Ajo Wy, as agreed with City of Tucson DOT. Contact Jose Ortiz (520) 837-6730 for guidance. It may be necessary to add a section detail at center of proposed entrance to sheet GD04 that shows the wedge curb, asphalt with sidewalk and multi-use path in the asphalt.
2) If there are any outstanding utility issues that affect the issuance of the floodplain use permit or SWPP, these issues must be resolved.

CONDITIONS OF PERMIT:
The following 14 conditions apply to permit T10OT00396 & T10BU00315:
1) Any outstanding utility issues must be resolved through the applicable utility companies. Contractor is responsible to coordinate all work near utility lines with the proper utilities prior to any site construction.
2) Call for Pre-construction meeting. Discuss during Pre-con or contact PDSD Engineering Inspector to confirm which drainage inspections may be needed. For a PDSD Engineering Inspection, call IVR (740-6970), or schedule with a Customer Service Representative at Planning & Development Services Department (PDSD), or contact PDSD Engineering at 837-4888, or schedule inspections online at: http://www.ci.tucson.az.us/dsd/Online_Services/Online_Permits/online_permits.html
3) This Drainage Plan and SWPPP are only for onsite drainage improvement construction associated with the Arizona State Veterans Home project as shown on the approved SWPPP and drainage plans (T10OT00396 & T10BU00315). See also Final Drainage Statement (April 2010) sealed 4/7/10 by Ramzi Georges, P.E.
4) A copy of stamped approved drainage plans, permits, and reports shall be kept at site at all times, until final drainage approval.
5) Interim erosion and sediment controls shall be maintained per approved SWPPP plans at downstream areas of disturbance limits. Prior to Final Approval all temporary controls shall be removed and permanent controls shall be placed.
6) Stabilized entrances shall be maintained at all active access entrances. Use of water trucks or other dust controls shall be utilized, and construction entrances shall be maintained to minimize sedimentation, dust issues, and tracking.
7) Storage or placement of construction material, vehicle staging / stockpile area, and/or designated concrete washout locations shall not be located within or near any basins, or water harvesting areas.
8) During construction activities, traffic control plans may be required if there is a safety hazard potential from construction activities conflicting with public traffic. Onsite traffic control plans shall minimize traffic hazard conditions between public access to buildings, and construction vehicles & activities.
9) Any revisions to Drainage Plan must be reviewed by PDSD Engineering Engineer and may include discussion, details, or a revised drainage report or plans. Call PDSD Engineering at 791-5550 to discuss any revisions.
10) For inspection purposes, post City of Tucson's "AZPDES - Posting Requirements" green sheet, or the NOI at construction entrance of site, at beginning of construction activities, and maintain this posted document throughout project construction.
11) All Operators shall be identified and have separate certification statements. Assure these certifications are available on-site.
12) Operator shall report to ADEQ any noncompliance (including spills) which may endanger human health or the environment. Operator shall orally notify office listed below within 24 hours:

13) Permitee shall notify PDSD when operation is ready for final drainage inspection. Final drainage approval shall not be given until all work, including installation of all drainage improvements and their permanent protective devices, and all erosion control measures have been completed in accordance with approved drainage plan and permit, and all conditions of permit are completed.
14) As-builts are required to be submitted under activity number T10OT00396 to PDSD permit clerks. They will be reviewed and then if acceptable, a copy will be provided to PDSD Engineering Inspector for final drainage inspections. For Permit Project Closure documentation, submit: 2 copies of as-builts, letter of completion for basin and other drainage improvements. As-builts shall include: constructed elevations for basin bleed pipe adjacent to utilities, and finished grades for drainage systems.
