CASE NUMBER: D09-0036 (T09BU01354) ACTIVITY NUMBER: T09CM03146 SUMMARY: Engineering Division of Planning & Development Services Department reviewed the building plan submittal and recommends approval. Please note that the following conditions shall be applied to the grading permit for the El Rio Community Health Center - El Pueblo Clinic. GRADING PERMIT CONDITIONS: The following 23 grading permit conditions apply to grading permit T09BU01354: 1) There shall be no construction vehicle traffic or parking, or stockpiling in the basin area. It is recommended that minimal construction activity in the basin to reduce risk of compaction to percolation soils. 2) Additional percolation basin testing may be required at post-construction inspection. Drain down time may not exceed 12 hours. If drain down time is not met, contact PDSD Engineering and Engineer of Record to determine if re-engineered basin bottom is needed; revised Grading Plan and drainage report may be required. 3) Provide heavy duty construction fencing around basin area. 4) Temporary material storage or vehicle staging / stockpile area, and designated concrete washout locations should not be located in or near basin or water harvesting areas. 5) Grading / Disturbance Limits shall be maintained and no disturbance outside of grading limits will be allowed. 6) Contact Permits and Codes (791-5100) for any questions regarding any City of Tucson right-of-way permit requirements. 7) Special inspection required for any toedowns or other subsurface structures. 8) Call for Pre-construction meeting once grading activity is expected to start. Onsite Traffic Control Plan design shall minimize traffic hazard conditions and minimize conflicts for public access to existing buildings, and construction vehicles & activities. 13) All Operators shall be identified and have separate certification statements. Assure these certifications are available on-site. Remaining signatures from the Operators must be on the SWPPP on the site copy of the SWPPP (exhibit or report) at or before commencement of construction. 14) Operator shall report to ADEQ any noncompliance (including spills) which may endanger human health or the environment. Washington, 5th floor (5515B-1) Phoenix, AZ 85007 Office: 602-771-4466; Fax 602-771-4505 15) Stabilized entrances shall be maintained at all active access entrances. 16) Use of water trucks or other dust controls shall be utilized, and construction entrances shall be maintained to minimize sedimentation, dust issues, and tracking. 17) For inspection purposes, post City of Tucson's "AZPDES - Posting Requirements" green sheet, or the NOI at construction entrance of the site, at beginning of construction activities, and maintain this posted document throughout project construction. 18) If grading construction is expected to last longer than the expiration date of the grading permit, contact PDSD to renew/extend the Grading Permit. If Final Grading Inspection has not been completed before the Grading Permit expires, and the permit has not been renewed, additional fees and reviews may be required. Request for grading permit extension may be submitted to Planning & Development Services Department (attn: Claudia Buford), prior to grading permit expiration date, for consideration of grading plan extension approval. 19) Any revisions to the Grading Plan must be reviewed by the PDSD Engineering Engineer and may include discussion, details, or a revised grading plan. 20) Call PDSD Engineering at 791-5550 to discuss any revisions. 21) After interim controls are removed, permanent erosion control are required on all disturbed areas, in the form of vegetative cover, gravel, a sized decorative rock, and/or other controls, or as shown on the plans. 22) The permitee shall notify PDSD when grading operation is ready for final grading inspection. Final grading approval shall not be given until all work, including installation of all drainage facilities and their permanent protective devices, and all erosion control measures have been completed in accordance with approved grading plan and grading permit, and any required reports have been submitted. 23) As-builts and letters of completion for the overall project are required. As-builts are required to be submitted under the activity number T09CM03146 to the permit clerks, for engineering review, and then if acceptable, a copy will be provided to the PDSD Engineering Inspector for final grading inspections. For Grading Permit project closure documentation, submit: one copy of original grading plans, 2 copies of as-builts, letters of completion (one for basin drain down time and function, and another letter for substantial completion of the project). Field reports, infiltration testing results, and other field observations will need to be submitted for final completion. As-builts shall include: inverts for inlets and outlets of drainage systems and basin, and general grades as determined by RLS, to show substantial completion. While the building plan footprints appear to match the unapproved version of the site plan footprint the building footprint on the site plan was not dimesioned and the overall dimensions could not be verified to match.
