PROJECT REVIEWER: Richard Leigh, DOT, Permits and Codes Section

RESUBMITTAL REQUIRED: Permits and Codes Section has reviewed the PIA Plan and does not recommend approval of the PIA Plan at this time. The following comments need to be addressed in the next submittal.

1. Include the plan number "I-2008-015" in title block of each sheet.

2. Include Development Services Department PIA activity number "T08000015" on cover sheet.

3. Include the development plan number on cover sheet.

4. Include Alvernon Way, Poe Street, and Holmes Street in plan's title.

5. Provide a detail for each type of proposed curb access ramp. Additionally, indicate the use of truncated domes as the detectable warning device.

6. Add the following general note: The contractor shall contact and/or coordinate any work with COT Streets & Traffic Maintenance Division, Electric Shop (791-3154) prior to relocation or placement of any street lighting or traffic facilities. Please Contact Ernie Encinas at (791-3154) for street lighting/traffic signal plan review.

7. Indicate that a 2' minimum set back per City of Tucson Development Standard 11-09.0 is required from the hinge point of any cut and fill slope on private property and the property line.

8. Scupper details have discrepancies between them. Review and revise appropriately, see redlines.

9. Sheet 2, Detail "New pavement & curb Detail": Show sidewalk and right-of-way limits.

10. Provide a profile of the proposed right turn lane on Alvernon Way. Include elevations for all ECRs, BCRs and angle points.

11. Relocate the curb access ramp on the north side of the proposed driveway entrance on Alvernon Way to be in alignment with the proposed ramp opposite of it.

12. The existing curb access ramp on the northeast corner of Poe Street and the southwest corner of Holmes Street must be brought into compliance with current ADA requirements.

13. Provide 6' wide sidewalk along Alvernon Way between Poe street and Holmes Street. The minimum sidewalk with along a roadway listed on the City of Tucson Major Streets and Routes Plan is 6' wide.

14. If the 3" waterline shown on Poe Street a private, it must be relocated onto private property.

04/18/2008 LANCE PETERSON DOT STREET MAINTENANCE REVIEW Denied We have reviewed this plan and have the following comments:

1. Sheet 2 - Detail A - Please verify the thickness of the rip rap pad. Typically the thickness si two times the D50.
2. Curb Access Ramps - Please call out the use of truncated domes for all curb access ramps.
3. Will there be any landscaping installed with this project? If so, please submitt the landscape plan to Paul Rosenboom at 4004 S. Park Ave. Bld #2 for review.
4. Please coordinate all work in the right of way with Paul Rosenboom at 791-5279 ext 234 to prevent damage to existing landscaping and irrigation.

Electrical

1. Sheet 3 - Please provide a note on the plan that calls for intercepting the existing conduit and re-routing it behind the new curb with new conductors.
2. Sheet 3 & 4 - Please provide a note to call Ed Serrias at 791-5279 ext 259 for inspection and instructions on the light pole and pull box relocation.
