04/03/2008 | ELIZABETH EBERBACH | ENGINEERING | REVIEW | Approved | April 3, 2008 FLOODPLAIN USE PERMIT NO: T08OT00753 ADDRESS: 1601 West Saint Mary's Road, Tucson Arizona 85745 PROPERTY OWNER: Carondelet St. Mary's Hospital and Health Center, Attn: Chuck Spencer 349-0131 (Property Management Dept, 2202 N Forbes Blvd Tucson AZ 85745-1412) The applicant is authorized to use a portion of the City of Tucson Special Flood Hazard Area, specifically described as: A portion of Parcel 116-15-088L, PTN FRAC LOTS 1 2 10 11 & 12 N2 SE4 LYG W OF SILVERBELL & S OF ST MARY'S & N OF ANKLAM 17.17 AC SEC 10-14-13, described as the temporary use parking area south of the hospital building, north of the existing parking structure, centrally located on the St Mary's campus; for the purpose of demolition as well as parking and drainage improvements for a hospital parking area. The applicant agrees to comply with City of Tucson Floodplain and Erosion Hazard Area Regulations, and are bound by the conditions stated below. Violation of any of the conditions stated or referred to below, as determined by the City Engineer, shall be grounds for revocation of this permit. Responsibility for renewal of this permit, if necessary, is with the applicant. Issuance of this permit does not constitute a waiver of other requirements contained in the Tucson City Code, Land Use Code, or any other Federal, State, or local regulation. No use other than as permitted herein is allowed. Additional uses allowed under City of Tucson Floodplain and Erosion Hazard Regulations require application for, review and approval of an additional Floodplain Use Permit. This floodplain use permit is issued with the following 10 conditions: 1) Except as approved by the City Engineer prior to the start of construction, development under this permit shall not obstruct, retard or divert the existing flow of floodwaters from or to adjacent properties. 2) This Floodplain Use Permit is issued concurrently with the Temporary Use Permit for demolition and improvements for this parking and drainage area. Copies of these documents shall be kept in a readily accessible location on site at all times. 3) An application/processing fee of $50.00 (submittal fee) must accompany each future request for work to be authorized under this floodplain use permit. Engineering study review fees of $150.00 may be imposed based upon the fee schedule in the current floodplain ordinance. 4) Contact Tucson Department of Transportation - Permits and Codes Section, Engineering Division at 791-5100 for questions relating to right of way permit information. This permit does not authorize any development, excavation or construction activity in the public right-of-way. 5) This project is located in a Zone A Special Flood Hazard Area as shown on FIRM panel 2226K. Currently a CLOMR has been issued by FEMA. 6) The associated improvements are with permit activity number T08CM01045. 7) The applicant attests to the fact that the plans and documents submitted represent all of the improvements that will be done at this location. Any and all other improvements, additions, repair, or construction of future permanent, freestanding walls or other structures shall have separate floodplain use permit applications submitted to this office for review and approval. 8) This permit may be renewed for up to one year, in the absence of Registered Plant authorization, at the sole discretion of the City Engineer or designated representatives. Renewal of this permit may require a new application, review and approval of that application by the City of Tucson, Development Services Department, Engineering Division. 9) This permit is specific to the temporary use improvements for the parking area south of the existing hospital building, north of the existing parking structure, in the central area of the St Mary's campus. A future parking structure is expected to be constructed at a later date. This permit does not apply to any structure and there is no substantial improvement impact. 10) Temporary interim erosion control devices including wattle or other sediment or erosion control BMP's shall be installed and maintained at downstream side of project including west side of parking area construction, until gravel is placed and accepted by City inspection. This Floodplain Use Permit will be automatically revoked and becomes invalid if conditions are not complied with, as set forth in this permit.
