07/03/2007 LANCE PETERSON DOT STREET MAINTENANCE REVIEW Denied We have reviewed these plans and have the following comments:

1. Various Sheets - Several new curb access ramps are not called out on the plan portion of the plans.
2. Sheet 4 - The new storm drain man hole is called out in the profile section and not in the plan section.
3. Sheet 4 - Station 15+50 - Plan section calls for a sidewalk scupper and the profile section calls for a catch basin.
4. Sheet 4 - The profile section shows an 11 foot high pek/berm. Is this the noise buffer, or what is this?
5. Sheet 7 - At the end of the pavement a concrete header should be installed to protect the pavement from eroding.
6. Sheet 7 - What signs will be installed to mark the end of the pavement on Hermans Road?
7. Sheet 8 - Station 13+40 - Plan section calls for a new storm drain manhole, but it is not shown.
8. Sheet 8 - Plan shows a D.G. pathway. Who will be responcible for the maintenance of this path?
9. Sheet 9 - Mermaid Cove - The inlet wall calls for rip rap. What is the rock size and thickness? Also, please call for filter fabric.
10. Sheet 9 - Sewerline "A" - Plan shows a berm at approximately station 11+50 and there is an 11 foot peak at approximately station 12+15. Are these the same thing and again is this the noise buffer, or something else?
11. Sheet 10 - Detail 3 - Who will be responcible for the maintenance of the landscaped berm?
12. Sheet 10 - Detail 9 - The detail shows a splash pad, but what is the thickness of the pad?
13. Will there be any landscapin installed with this project? If so, please submit the plan to Paul Rosenboom at 4004 S. Park Avenue, Bld #2, for review. Please remember that private irrigation line can not be placed under public roadways.
14. Will the home owners association be responcible for the maintenance of the common areas, including the erosion protection coming off of the streets?

2. Please submit your landscape and irrigation plans for the common areas. What I am looking for on the irrigation plan is the location of irrigation lines. NOTE: private irrigtation lines can not be placed under public roadways. (Blue Stake Law)

Resubmittal Required: Traffic Engineering does not recommend approval of the PIA Plan; therefore a revised PIA Plan is required for re-submittal.

The following items must be revised or added to the plan.

1. Include a response letter with the next submittal that states how all comments have been addressed.

2. See redlines on the submitted drawings and address the additional redline comments. During the next submittal please re-submit the roll of redlines for back checking purposes.

3. Install R1-1 (Stop Sign) with street names at the proposed roadways entering this development off Hermans Road per PCDOT & TDOT Signing Manual (2002) Chap 5-11, 5-12, and Chap 9-2.

4. Install R2-1 (25MPH Speed Limit Sign) at the proposed roadways entering this development off of Hermans Road per PCDOT & TDOT Signing Manual (2002) Chap 9-10.

5. Parking is not allowed in cul-de-sacs from PRC to PRC. So indicate on the plans by showing no parking signs on the plans. (DS 3-01.0 fig 20)

6. Provide a detail of the No Parking sign to be used. Attached to the red line plan set is a copy of the detail the city has accepted.

7. Place a "NO OUTLET" (W14-2) sign on Hermans Road 50 feet east of Talaco Trail for eastbound motorists.

8. Place 3 Object/Hazard Markers (OM4-3), equally spaced, at the end of Hermans Road.

9. Place the PIA and CDRC Case number on the cover sheet.

10. Add the Signing General Notes found in the PCDOT & TDOT Signing Manual (2002) (Chapter 1, Sheet 12) to the Plans.

11. Add the following Pavement Marking/Striping General Notes to the Plans:

'ALL STREET STRIPING & SIGNAGE TO BE COORDINATED WITH C.O.T. TRAFFIC ENGINEERING CALL 791-4259 A MINIMUM OF 48 HOURS PRIOR TO THE INSTALLATION OR REMOVAL OF ANY SIGNAGE OR STRIPING. ALL REQUIRED STRIPING AND SIGNAGE WILL BE INSTALLED AT THE CONTRACTOR'S EXPENSE. NEW STRIPING TO BE THERMOPLASTIC MATERIAL. USE 2" SQUARE-TUBE POST AND SQUARE TUBE POST FOUNDATIONS FOR SIGNING. 07/26/2007 Blanca Espino DOT ENGINEERING REVIEW Denied PIA Plan
1. Please include a detail letter of explanation with the next submittal indicating where and how revisions were made. Single word responses are unacceptable. Include plans/redlines from previous plan submittal.

2. The PIA plans shall be designed in reference to the PIA Guidelines & Checklist. Comply with all Development Standards & Codes for road improvements. Drainage and Soils Reports must have information coinciding with the approved Tentative Plan and the proposed PIA Plans.

3. The approved Drainage Report, Geotechnical Report, and Tentative Plat must be submitted for upcoming reviews, otherwise submittals will be denied.

4. A Geotechnical Report is required for submittals and must include pavement section and reference soil recommendation for public roads.

5. Maps and Plats, Book and Page should be noted on cover sheet of subdivision street improvement plans. Plat must be recorded before submittal of mylars for final review and approval.

6. Sheet 1, In title block, add assigned I-Plan Number, I-2007-026 to all sheets.

7. Sheet 1, Indicate assigned DSD activity number T07PI00019 above title block. See redlines.

8. Sheet 2, Indicate in General Notes; Pavement Section Design documentation, are in accordance to the approved Geotechnical Report, Report Name and Firm Name, Project Number.

9. Sheet 2, General Note #20 shall read; The PIA Inspector shall verify that the "saw cut" line is located within competent pavement. Adjustments to the plan shall be documented and As-built; a one foot minimum shall be provided.

10. Sheet 2, Indicate in General Notes; Contractor shall obtain all other permits as required by Local, State and Federal requirements.

11. Sheet 2, General Note #39 indicate Floodplain Use Permit No. T07BU01190 has been issued for this project. The FUP number is a grading activity number. FUP activity numbers start with T07OT0xxxx. Correct and revise.

12. Indicate All Easements, dedications, right-of-way should be depicted in plan view and labeled with recordation information (docket/page or plat book/page). Apply this information where there is an Easement of any kind or where it reads "Dedicated by Final Plat". Correct and revise.

13. Sheets 3 thru 9 require the following information; Roadway profiles (Left, Center & Right) are required for new roadways, including widening projects, deceleration, acceleration and turn lanes. Refer to COT/Engineering Division Active Practices Guidelines No. 8-1552-002. Correct and revise.

14. Sheets 3 thru 9; Stationing for Plan and Profile view shall be aligned. See redlines. Correct and revise.

15. Relocate all HCS/Sewer Data Tables on separate sheet following details.

16. Provide Curve and Line Data Table. Correct and revise.

17. Sheet 10, Details 1 & 2; Hinge points of cut and fill slopes must have a minimum setback of two feet from right-of-way. (Slope on private side). D.S. 11-01.9.0 (9.2, 9.3 & Figure 2)

18. Curb access ramps on a continuous roadway/sidewalk shall have a minimum of 300 feet distance between ramps. Ramps opposite of one another must line up. All curb access ramps should have the 6-inch curb at back of ramp. Indicate a dimensioned detail for mid-block ramp.

19. A dimensioned detail for each type of curb access ramp to be built as part of the project must be shown. Truncated Domes required.

20. A dimensioned detail for each type of drainage structure in the right-of-way to be built as part of the project must be shown.
