1. Please include a detail letter of explanation with the next submittal indicating where and how revisions were made. Single word sentences are unacceptable. Include plans/redlines from previous plan submittal. Submit Drainage & Geotechnical Reports.

2. Maps and Plats, Book and Page should be noted on cover sheet of subdivision street improvement plans. Plat must be recorded before submittal of mylars for final review and approval.

3. In title block, add assigned I-Plan Number, I-2007-012 to all sheets.

4. Indicate Administrative address on cover sheet. Correct and revise.

5. Remove all Sewer Notes and information other than manholes.

6. Indicate all street and drainage structures.

7. Attached is a copy of the PIA Guidelines and Checklist that shall be applied to all PIA plans.

03/09/2007 LANCE PETERSON DOT STREET MAINTENANCE REVIEW Denied We have reviewed these plans and have the following comments:

1. Is the perferations in the underground detension system to allow water to perk out? Will these pipe hold water? Staning water is a concern with the West Nile Virus.
2. Sheet 3 - What asphalt mix design will be used? (mix #2)
3. Sheet 5 - The entances off of Fairview Buisness Park Place do not show curb access ramps.
4. Will there be any landscaping with this project? If so, please submit a landscape plan to Paul Rosenboom at 4004 S. Park Ave. Bld #2.
5. All work in the right of way needs to be coordinated with Paul Rosenboom at 791-3154 ext 234 to prevent damage to existing landscaping and irrigation.
