Engineering Division does not recommend approval of the site plan at this time. Please address the following review comments prior to the next submittal.

Additional Comment: Verify that each right-of-way has both the existing and future right-of-way labeled and dimensioned correctly. If the right-of-way dimension is a ½ right-of-way, identify it as a ½ right-of-way. For any dimensions that are both existing and future, label accordingly.

STORMWATER POLLUTION PREVENTION PLAN:
42. Arizona Pollutant Discharge Elimination System (AZPDES) requirements are applicable to this project. Stormwater Pollution Prevention Plans (SWPPP) and text addressing stormwater controls for all areas affected by construction activities related to this development will be required with the re-submittal. For further information, visit www.adeq.state.az.us/environ/water/permits/stormwater.html. A SWPPP submittal will be required along with the grading permit. Every Construction General Permit required a separate SWPPP.

43. Grading plan will require full SWPPP - see ADEQ checklist to ensure that all items have been includedd in report.

SWPPP Comments:

1. Provide location map on the SWPPP and indicate and label "receiving waters".
2. Delineate grading limits on plan view and assure that they match what is indicated on the grading plan
3. Provide contour lines at minimum 2' interval on plan view and call out contour interval on plan
4. Assure sufficient erosion and sediment controls are set at the downstream boundaries of disturbance areas. Assure that interim controls are placed within grading limits.
5. Provide a traffic control plan for activities taking place during construction. Provide this plan within SWPPP or along with the grading submittal. Locations of construction access and signage must be indicated on plan view of the traffic plan.
6. It appears that additional stabilized construction access area for southwest parking area and entrances to Silverlake and entrance to Park Ave. On Silverlake clarify that entrance is for construction only access, if it does it will need to have stabilized construction area access.
7. For detail 1 on SWPPP, it is recommended that the notation in regards to washed gravel backfill be removed. This practice is not normally performed in the field and could potentially cause concentrated flow and erosion where breakout occurs. In addition, add flow arrow to detail 1.
8. For NOI provide completed form that has been faxed to ADEQ, once grading plan submittal has been provided.
9. Provide operator certification, at minimum (1) operator must complete and sign certification statement.
10. Revise sequence of activities on sheet 3, to include traffic control plan during construction activities.
11. Provide contact number and information for the event of a spill.

Provide a revised site plan, traffic control plan, and SWPPP that address the comments provided above. Include a comprehensive response letter addressing in detail responses to all the above comments.

Submit grading permit application with next submittal.
