03/21/2007 ELIZABETH EBERBACH ENGINEERING REVIEW Denied TO: Rose Duarte-Pina, TetraTech
SUBJECT: 1300 South Lost Starr Drive Gated Entrance Grading Review
LOCATION: T14S, R13E, Section 20
REVIEWER: Elizabeth Eberbach
ACTIVITY NUMBER: T07BU00178

Summary: Engineering Division has reviewed the submitted grading review items: the copy of the entrance grading plans, Quail Hollow paving plans, other supporting documents, including the proposed access easement legal description and exhibit. The plans are denied until the following items are addressed.

GRADING COMMENTS:
1) As-built drainage structure adjacent to proposed improvements indicate a 54-in CMP was installed where a 11-ftx4-ft con-arch was originally proposed on the Quail Hollow improvement plans. It is necessary to assure all weather access along Lost Starr Drive. The WSEL for 100-year jurisdictional flow at existing culvert crossing is necessary to determine all weather access.
a) Provide copy of drainage calculations for existing 54-in RCP crossing.
b) Label existing 54-in CMP invert elevations on sheet 2.
c) Indicate water surface elevation at Lost Starr Drive on sheet 2.
d) Delineate 100-year floodplain limits on sheet 2.
e) Assure proposed elevations of the road pavement provide all-weather access for any over topping condition during 100-year event.
f) A floodplain use permit application may be necessary if the project encroaches the 100-year floodplain.
2) Provide a copy of docket and page of Lost Starr Access Easement with the legal description and exhibit.
3) Add the following notes to the cover sheet:
a) If grading construction is expected to last longer than the expiration date of the grading permit, contact DSD to renew/extend the Grading Permit. If Final Grading Inspection has not been completed before the Grading Permit expires, and the permit has not been renewed, additional fees and reviews may be required.
b) The permitee shall notify the DSD when the grading operation is ready for final grading inspection. Final grading approval shall not be given until all work, including installation of all erosion control measures have been completed in accordance with the approved grading plan and grading permit.
4) Provide additional pavement spot elevations around proposed medians to minimize potential ponding and assure positive drainage.
5) Contact Fire Department regarding application for fire box for proposed gate, enclosed with the plan package.

Submit 4 copies of the revised grading plan with supporting documents including drainage statement, floodplain use permit application if applicable, and docket and page of new access easement. I can be reached at 791-5550, ext.2204 if you have questions.
