5. Sheet 1, Add note to General or Paving Notes: Signage and striping should be coordinated with Traffic Engineering and installed at no expense to the public. Call Traffic Engineering at 791-4259 prior to the installation or removal of striping and signage.

6. Sheet 1, Add note to General or Paving Notes: Engineer of Record must provide written documentation that all utilities within the right of way have been cleared for construction.

21. A profile is required indicating existing and new right-of-way, curbs and utility systems. Show existing grades at both right-of-way lines and centerline alignment. The new grades shall be shown at centerline and both top of curb and gutter, or at edge of pavement if no curb is to be constructed. Indicate elevations, control lines, stationing, percent of grade and all necessary related profiles pertaining to existing sanitary sewers, storm sewers and utilities.

22. A 2% (max) cross slope must be provided across roadway/PAAL at pedestrian crosswalks.

23. All new development shall provide 5-foot wide sidewalks along the entire length of street starting at a minimum of 2-feet from the back of curb. D.S.3-01.2.7.B & D.S.3-01.4.1.A.1.a Correct and revise.

24. A minimum of 2-foot buffer must be provided between top of slope and sidewalk (natural slopes). Slopes greater than 4:1 and deeper that of 2-foot require barricade railing.

25. Terminal curb sections at end of vertical curbs are required. Correct and revise. See redlines.

26. Indicate a dimensioned detail for each type of curb access ramp to be built. Truncated Domes required.

27. It appears that the existing contour lines direct the street runoff into Carmax parking lot through the ingress/egress. The approved Drainage Report does not indicate how the new vertical curb will accept, divert or block the street runoff from its existing state. Demonstrate and justify the impacts for the existing and proposed drainage design.

28. Ingress/egress on the eastside of the parcel is to remain in its existing state. On undivided roadways, access on both sides of the road shall be aligned. T.A.M.G.4.2

29. It is required to set & place new curb at a set dimension from centerline of right-of-way. Correct and revise.

30. Submit a copy of the Soils/Geotechnical Report by Golder Associates Inc., Ref No. 043-1841 Dated April 21, 2005.

31. Based on the approved Mylars, DOT Permits and Codes will conduct a PIA pre-construction meeting with the Developer, Engineer of Record, and Contractor, and issue the approved permit application. Based on the approved permit application, DSD will collect applicable fees (5% of total construction costs) and issue the Excavation Permit to work in the Public right-of-way.
