1. Sheet 5 of 6 - The plans call for tying into existing water. There is no existing water source.
2. Sheet 6 of 6 - Detail #5 - Need to specify Sch 80 PVC unions.
3. Sheet 6 of 6 - Detail #9 - It appears that the first detail number 9 should be labeled number 6. Also, you need to use multiport emitters.
4. Sheet 6 of 6 - Detail #8 - This detail needs to meet C.O.T. specifications.

3. All text height is to be 0.12 (1/8 inches). Correct text height for AMEC address & numbers on all sheets.

4. Sheet 1, Remove text on plan view. See redlines.

5. Sheet 2, Indicate a dimensioned Right-of-Way cross section detail of Houghton Road showing Existing, New and Future pavement structural information. See redlines.

6. Sheet 2, Detail A-A, Indicate street slope, property line / right-of-way limits, arrows should point to objects that are being called out, remove thickened edge detail, saw cut is 2-foot minimum, new pavement section should read 1. See redlines. Correct and revise.

7. Sheet 2, Detail B-B, Indicate street slope, property line / right-of-way limits, remove thickened edge detail, saw cut is 1-foot minimum, call out new or existing vertical curb. See redlines. Correct and revise.

8. Sheet 3, Remove word "Grading" from sheet heading "Paving and Grading Plan". Correct and revise.

9. Sheet 3, Stationing on plan and profile sheets should increase from Left to Right regardless of which way the north arrow points. Graphically indicate stationing on Plan view. See redlines. Correct and revise.

10. Sheet 3, Indicate street slope for proposed area and show graphically. 2% minimum. See redlines. Correct and revise.

11. Sheet 3, Relocate Curve Data Table and Details below Profile.

12. Sheet 3, Pavement thickness and surface treatment should match after saw cutting at a minimum of 2-foot into existing pavement. Correct and revise.

13. Sheet 3, Elevations at STA. 92+10.76 and 92+55.74 show a big difference. Correct and revise.

14. Sheet 3, Indicate a dimensioned detail for sidewalk area along Stations 93+00 to 91+00. Sidewalks must be physically separated from any vehicular travel lane by means of curbing, grade separation, barriers, railings, or other means, except at crosswalks. Terminal curb sections/sidewalk at end of curbs and concrete header at end of pavement are required. Correct and revise.

15. Sheet 3, Indicate 5 of compaction for Existing pavement and Pavement Section 1.

16. Indicate a dimensioned detail of curb access ramp to be built. Truncated Domes are required. Correct and revise.
05/26/2006 DALE KELCH DOT TRAFFIC REVIEW Denied Traffic Engineering REJECTS this PIA submittal:

1. I missed it first time, remove all reference to crosswalks across driveways. A .pdf submittal showing the removal of the crosswalks will satisfy this correction.
