Silverlake Road Lot #: 33 Property Owner: Villa Capistrano, LLC Permit Summary T06OT01747, Lot #: 33, 1325 W. Silverlake Road: BFE (NGVD 1929) = 2379.3 Feet. The applicant is authorized to use a portion of the City of Tucson the property described as: Cottonwood Park that PT of Lot 114 LYG N of A LINE 1421.11' S of Silverlake Road, Parcel 11810134B, T14 R13 S22, 23, &26. 1. This permit is for the purpose of re-setting a manufactured home within a FEMA floodplain. 2. The lowest structure member (LSM) of the building on the property under this permit shall be placed a minimum of one foot above the base flood elevation (BFE), NGVD 1929 as determined by an Arizona Registered Land Surveyor (RLS) and certified on an Elevation Certificate "FEMA Form 81-31, February 2006". 3. All service equipment, including electrical panel, air conditioner pad, and/or water heater, must be elevated a minimum of one foot above the BFE. The elevation of the lowest machinery or equipment servicing the structure must be recorded on the Elevation Certificate describe in 2 (above). 4. The manufactured home must be anchored and certified on a "Certificate for Manufactured Home Anchoring and Utilities Location" form by an Arizona registered professional civil engineer in accordance with the City of Tucson Floodplain Ordinance # 7407, Section 26-9. 5. The original Elevation Certificate and the original Certificate for Manufactured Home Anchoring and Utilities Location, must be completed and returned to City of Tucson, Development Services Department, Engineering Division, on the 1st floor of the County/City Public Works Building at 201 N. Stone Avenue, Tucson Arizona, prior to occupancy of the structure. 6. This Floodplain Use Permit shall be kept in a readily accessible location on site until installation activities are completed and required documentation described in # 5 (above) is submitted and approved by DSD. 7. This Floodplain Use Permit will be automatically revoked and becomes invalid for non-compliance to conditions set forth in this permit. 8. This Floodplain Use Permit expires 180 days from the issue date.
