SUBJECT: Carmax-SWPPP T06BU01096 (First Review)
T13S, R13E, Section 23

RESUBMITTAL REQUIRED: SWPPP as a supplement to the already submitted Grading Plan.

The Stormwater Pollution Prevention Plan (SWPPP) cannot be approved as submitted. Please address the following review comments prior to the next submittal.

SWPPP:

1. It is acknowledged in section 1.9 (page 4) 'Notice of Intent' that the NOI form must be signed by the operator and submitted to ADEQ and in section 2 a NOI has been prepared by Ned Sparkman, however all parties involved with "Control Over Day to Day Operations." Needs to provide a NOI. Identification of the Operator with Operational "Control Over Construction Plans and Specifications", must also be provided. (Part IV.C.1) Where an operator is unknown (such as a yet to be determined general contractor, engineer) provide an empty NOI form for the identity to be supplied later. In addition revise section 1.9 to state that NOI form must be signed by the operator(s) and submitted to ADEQ.

2. Each operator is responsible for submitting a completed NOI to ADEQ and a copy to the City of Tucson. Provide copies of the NOIs in the narrative. (Part IV.F)

3. Provide a sheet index on the first sheet.

4. It is difficult to determine the location of the silt fencing. Please revisit and illustrate all location of the fencing.

5. Place titles of each sheet in the title block to clarify the differences between each sheet.

6. Sheet 2: A small plan view drawing of the site needs to be removed from the drawing.

7. Revisit the LEGEND and crosscheck the items listed in the legend against the plan view. The following items are in the legend but are not shown in the plan view. (catch basins, curb opening, existing watershed, and the 100-yr floodplain)

8. Legend on each sheet differs from one another, consider using a centralized legend on one sheet, and refer back to that legend to avoid confusion.
