2) Sheet 1 - A note states that the striping notes are on sheet 4, but the striping notes are on sheet 2 and the striping plans are on sheet 4. It would be a good idea to have them together.
3) Sheet 2 - There are two cross sections labeled "G". Please clarify.
4) Sheet 2 - Details A, B, & C should be referenced as A/2, B/2, & C/2 since they are shown on sheet 2.
5) Sheet 2 - Cross Section H calls out detail D/3 and should be B/2.
6) Sheet 2 - Second Cross Section G calls for detail F/3 and should be C/2.
7) Sheet 3 - There are call outs for D/3, E/3, & F/3. Please clarify.

RESUBMITTAL REQUIRED: PIA Plan and Drainage Report.
The Engineering - Permits and Codes Section has reviewed the PIA Plan and Drainage Report and does not recommend approval of the PIA Plan at this time. The Drainage Report is reviewed for PIA purposes. The attached comments are based on the information provided and is not meant as an all inclusive list of items that must be addressed but a check for general conformance to City Standards. The Engineer of Record (the engineer preparing and certifying the plans, in this case Rick Engineering) is responsible for the plans/design meeting all applicable Standards, Codes, Ordinances, and for any issues errors and/or omissions. The following review comments need to be addressed in the next submittal.

PIA Plan
1. Please include a response letter with the next submittal that states how all comments have been addressed. Include plans/redlines from previous plan submittal.

2. Maps and Plats, Book and Page should be noted on cover sheet of subdivision street improvement plans. Plat must be recorded before submittal of mylars for final review and approval. Correct and revise.

3. Add note to Striping Notes; Signage and striping should be coordinated with Traffic Engineering and installed at no expense to the public. Call Traffic Engineering at 791-4259 prior to the installation or removal of striping and signage.

4. Add note to General or Paving Notes: Engineer of Record must provide written documentation that all utilities within the right of way have been cleared for construction.

5. Sheet 1 of 4, Correct and revise Subheading SEE SHEET 4 FOR STRIPING NOTES to SEE SHEET 2 FOR STRIPING NOTES.

6. Sheet 2 of 4, #1, correct and revise 2002 Edition to read 2003 Edition.

7. Sheet 2 of 4, Correct and revise detail callout letters.

8. Sheet 2 of 4, Indicate existing pavement dimensions for the details calling out existing pavement. See redlines.

9. Sheet 3 of 4, Identify the line with three dots and indicate in legend.

10. Sheet 3 of 4, Correct and revise detail callout page number.

11. Sheet 3 of 4, The FS 95.5 on STA 18+03.84, 65.52' RT, does not match the elevation that is noted at elevation 94.0. Correct and revise.

12. Indicate for 22nd Street and Old Spanish Trail if proposed street grades and pavements meet the approved Geotechnical Report recommendations. Saw cut line shall not be located along wheel path.

13. Please add the following checked notes to the General Paving Notes of the PIA Paving Plans: The Engineer of Record shall submit to the City for acceptance any changes to the approved plans prior to construction. Additionally, the Engineer of Record shall certify all changes meeting all applicable standards, codes, and ordinances.

14. If applicable, the contractor shall contact and/or coordinate any work with COT Streets & Traffic Maintenance Division, Median Island Maintenance Section (791-5279, ext. 234) prior to disturbing any COT/DOT landscaping or irrigation systems. Additionally, the contractor shall repair or replace any disturbed COT/DOT landscaping or irrigation systems, including replacement of any damaged plants.

15. Utility Clearance Letters (DSD PIA coordinator to track and collect letters from EOR). See attached.

16. Based on the approved Mylars, DOT Permits and Codes will conduct a PIA pre-construction meeting with the Developer, Engineer of Record, and Contractor, and issue the approved permit application. Based on the approved permit application, DSD will collect applicable fees (5% of total construction costs) and issue the Excavation Permit to work in the Public right-of-way.

17. Permits and Codes, and Traffic Engineering will conduct their respective inspections in accordance to City approved plans and applicable City Standards. Inspection results will be provided to Development Services Department for their record tracking and information. Inspection request / inquires will be processed through the Development Services Department PIA coordinator.
09/21/2005 DALE KELCH DOT TRAFFIC REVIEW Denied Traffic Engineering REJECTS this PIA submittal:

1. The boxed areas on sheet 1 in the project map indicate that the highlighted areas are on sheet 2. These areas are on sheet 3.

2. There are 2 section details called out as section "G" on sheet 2.

3. Identify the streets in question in sections G, H and the 2nd G.

4. Several pavement section bubbles in the above mentioned sections call out the subsequent sections as being on sheet 3. The pavement sections are on sheet 2.

5. The section of station 12+73 calls out a section F. Where is section F in the plans?

6. Show vertical curb in the sections.

7. Sheet 3 calls out sections D, E, F. Where are these sections in the plans?

8. Any area where there will be new turn lanes will have vertical curbing.

9. Return radii at access points will be of vertical curb vice concrete headers.

10. Taper is misspelled as "tapper" on sheet 4.

11. Relocate existing R4-4 at station 14+98 to station 12+60 (+/-).

12. Relocate existing W4-7 (you have this sign called out as W20-8) from station 13+91 to 13+00 (+/-).

13. Install 2 new R3-5R signs on each turn lane. One sign to be located at the angle point the other located at the BCR.

14. Install new R3-5R sign in raised median diverter island.

15. Identify the sign depicted in the raised median diverter island.

16. Relocate existing R2-1 on Old Spanish Trail from station 13+38 to station 14+20 (+/-).

17. Install new R4-4 at beginning of taper for new turn lane on Old Spanish Trail.

18. Edge line for exiting traffic on 22nd should be WS6 vise WS12.

19. All stop bars are to be WS12 vise the 18" called out.
