09/20/2005 LANCE PETERSON DOT STREET MAINTENANCE REVIEW Approv-Cond I have reviewed these plans and have the following comments:

1) Sheet 1 - Note 4 - This note calls out Asphalt Concrete but is related to Portland Cement Concrete (Class "S").

2) The plan calls for 2" of asphalt concrete over 4" of aggregate base course. What is the asphalt mix to be used?

09/30/2005 DALE KELCH DOT TRAFFIC REVIEW Approved Traffic Engineering recommends APPROVAL of this PIA submittal.

Please add a general note on the mylars to read "All non-signalized intersection street names must have E-W block number addresses for E-W roadways and N-S block number addresses for N-S roadways.” I missed this on the 1st review.

The consultant should note, that contrary to the response letter for TRAFFIC's previous comments, the POSTED speed is not listed in general note 1 on either the 1st submittal or this submittal. The posted speed does show up in the street sign legend on sheet 1/8 for this submittal.
