SUMMARY: The TI plans, building review documents, floodplain use permit application, valuation of the improvement, and location map were received and reviewed by Engineering Division. Engineering recommends approval of the TI plans and a Floodplain Use Permit will be issued.

CONDITIONS OF PERMIT:
1) As stated in prior correspondence, any improvements to the structural building for this project are considered a substantial improvement to an existing floodprone structure if the value of this new improvement is 50% or more than the value of the existing structure in present day value. If the project is determined to be a substantial improvement, and, if it is determined that the lowest floor elevation is below the regulatory floor elevation, a design submittal, and review and acceptance of the submittal for floodproofing will be required and shall conform to City of Tucson and FEMA floodplain development requirements.
2) Cumulative values shall be accounted for each building structure, so that tracking of the 50% status is current.
3) All future St Mary's building structures 2005 shall be submitted with a Floodplain Use Permit application. It shall be the responsibility of Carondelet Health Network, or its authorized agent, to supply the following with future Floodplain Use Permit Applications:
a) An 8-1/2" x 11" map locating improvement on the St. Mary's campus
b) Description of improvement
c) Provide permit activity number(s)
d) Title of improvement
e) Type of assessment used for valuation
f) Cumulative value of the building structure
g) Value of improvement in U. S. Dollars
h) A calculated percent improvement
4) A Floodplain Use permit with other conditions will be issued for this TI.
