SUMMARY: The TI plans and building review documents, described as 3rd Floor Inpatient Rehabilitation area of the South Wing hospital building, were received and reviewed by Engineering Division. The Floodplain Use Permit has been issued with the demolition plans with the floodplain use permit application T06OT00164, that included valuation of the improvement and location map, for the purpose of tenant improvements to an existing, non-conforming floodprone structure. Engineering recommends approval of the TI plans.

CONDITIONS OF PERMIT:
1) The associated improvements are with permit activity number T05CM06065 (Demo / TI plans), T06OT00164 (Floodplain Use Permit).
2) Any improvements to the structural building for this project are considered a substantial improvement to an existing floodprone structure if the value of this new improvement is 50% or more than the value of the existing structure in present day value. If the project is determined to be a substantial improvement, and, if it is determined that the lowest floor elevation is below the regulatory floor elevation, a design submittal, and review and acceptance of the submittal for floodproofing will be required and shall conform to City of Tucson and FEMA floodplain development requirements.
3) Cumulative values shall be accounted for each building structure, so that tracking of the 50% status is current.
4) All future St Mary's building structures 2006 shall be submitted with a Floodplain Use Permit application. It shall be the responsibility of Carondelet Health Network, or its authorized agent, to supply the following with future Floodplain Use Permit Applications:
a) Value of improvement in U. S. Dollars.
b) Type of assessment used for valuation.
c) Cumulative value of the building structure.
d) A calculated percent improvement.
e) Title of the improvement.
f) Description of improvement.
g) Provide 8-1/2" x 11" map locating improvement.
h) Provide permit activity number(s).
5) The applicant attests to the fact that the plans and documents submitted represent all of the improvements that will be done to the existing building, and that all additions, improvements or repairs to the building are included in the construction plans and/or documents herewith. No other improvements, repairs, reconstruction, additions or remodeling have been made to the structure that is not indicated in the attached plans and/or documents.
6) This permit is specific to the improvements for the rooms for the building specified as the construction of the interior improvements for the 3rd floor area of the south wing as shown on this plan. It shall not apply to any other structure or property, whether attached or unattached, not specified under this permit.
7) Copies of the improvement plans/permit documents shall be kept in a readily accessible location on site at all times.
