Speedway Blvd. Grading plan T05BU01122 (First Review) T14S, R15E, Section 26 RESUBMITTAL REQUIRED: Grading Plan, Drainage Report and SWPPP The Grading Plan (GP), Drainage Report (DR) and Stormwater Pollution Prevention Plan cannot be approved as submitted. Please address the following review comments prior to the next submittal. Grading Plan: 1. Please include a response letter to the comments along with the corrected copies of the GP. 2. The GP, DR and SWPPP cannot be approved prior to the approval of the Site plan. 3. Provide bollards and depressed curb in front of the basin access ramp. 4. Please show the proposed roof drainage patterns, 100% of the 10-year flow must be conveyed under the sidewalks. Please provide supporting calculations to demonstrate compliance with D.S. 3-01.4.4. If the location(s) of the roof scuppers have not yet been decided, add a general note indicating sidewalk scuppers (per COT/SD) will be used when the roof scuppers locations have been designed. Per previous comment. 5. Water havesting is not allowed in the right-of-way. A permit or a private improvement agreement will be necessary for any work performed within the Right-of-way. Contact Permits and Codes at (520) 791-5100 for permit information. 6. A flood plain use permit is required. Per previous comment. 7. Delineate 100-year W.S.E.L., Floodplain and erosion hazard setback. 8. Cut or fill slopes require a 2' minimum setback from property line. Show on all details. Per pervious comment. Drainage Report: 1. Please include a response letter to the comments along with the corrected copies of the DR. 2. Add the basin maintenance responsibility note per S.M.D.D.F.M. 2.3.1.6 C 1 and 2 to the DR. Stormwater pollution prevention plan: Operators Identify all operators for the project site, and the areas over which each operator has control. (Part IV.C.1) Indicate in the SWPPP the name(s) of the party(ies) with operational control over project specifications (including the ability to make modifications in specifications). (Part IV.B.2.c) Indicate in the SWPPP the areas of the project where the operator has operational control over project specifications, including the ability to make modifications in specifications. (Part IV.B.1.b) Indicate in the SWPPP the name(s) of the party(ies) with day-to-day operational control of those activities necessary to ensure compliance with the SWPPP or other permit conditions. Provide a table for recording the names and responsibilities for each party responsible for activities necessary to ensure compliance with the SWPPP or other permit conditions. (Part IV.B.1.d) Indicate in the SWPPP the areas of the project where each operator has operational control over day-to-day activities. (Part IV.B.2.c) Stabilization Practices 1. The operator must maintain the following records as part of the SWPPP. Provide pages to record the required information. (Part IV.D.4.c) a. Dates when major grading activities occur. (Part IV.D.4.c.i) b. Dates when construction activities temporarily or permanently cease on a portion of the site. (Part IV.D.4.c.ii) c. Dates when stabilization measures are initiated and completed and reasons for delay, if applicable, under IV.D.4.b. (Part IV.D.4.c.iii) 2. Describe measures to minimize the generation of dust. (Part IV.D.8.b) 3. Describe measures to sufficiently stabilize soil at culvert locations to prevent the formation of rills and gullies during construction. (Part IV.D.8.e) Permit Related Records 4. Include a copy of the general permit and the NOI that was or will be submitted to ADEQ. If other agreements with any state, local, or federal agencies exist that would affect the provisions or implementation of the SWPPP, include copies of those agreements. (Part IV.F) Inspections 5. Add a note stating that the operator(s) shall report to ADEQ any noncompliance (including spills) which may endanger human health or the environment. Include a certification and signature for each operator in accordance with Part VII.K. (Part IV.J.1) 7. All NOIs must be signed and certified as follows: a) For a corporation: By a responsible corporate officer. For the purpose of this Part, a responsible corporate officer means: i) A president, secretary, treasurer, or vice-president of the corporation in charge of a principal business function, or any other person who performs similar policy or decision-making functions for the corporation; or ii) The manager of one or more manufacturing, production, or operating facilities, provided, the manager is authorized to make management decisions which govern the operation of the regulated facility including having the explicit or implicit duty of making major capital investment recommendations, and initiating and directing other comprehensive measures to assure long term environmental compliance with environmental laws and regulations; the manager can ensure that the necessary systems are established or actions taken to gather complete and accurate information for permit application requirements; and where authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures. b) For a partnership or sole proprietorship: By a general partner or the proprietor, respectively; or c) For a municipality, state, federal, or other public agency: By either a principal executive officer or ranking elected official. For purposes of this section, a principal executive officer of a Federal agency is the chief executive officer having responsibility for the overall operations of a principal geographic unit of the agency (e.g. Regional Administrators of EPA.).
