Indicate site address. 2. Indicate a location map (3" =1 mile) and north arrow. 3. All lettering and dimensions shall be the equivalent of twelve (0.12") point or greater in size. The purpose of this requirement is to assure that all lettering is legible when reviewed and will maintain that legibility when reproduced and photographically reduced (microfilmed) for record-keeping purposes. 4. Indicate cut and fill quantities in cubic yards. 5. Indicate building roof slope, downspouts and sidewalk scuppers. Add note, "All roof spouts will be routed under any adjacent sidewalk." Graphically indicate downspouts and sidewalk scuppers. 6. Indicate adjacent property zones. 7. Use a different line weight to indicate contour elevations. Numbers are too light. 8. From Loren Makus E.I.T., Engineering Division RE: Stormwater Pollution Prevention Plan for T05BU00955 The Stormwater Pollution Prevention Plan (SWPPP) does not meet the minimum requirements of the AzPDES construction general permit. Revise the SWPPP in accordance with the following coments: 1. Remove all references to Pima County Transportation Department as a party to this permit. 2. Explain how the placement of sediment logs at the drainage channel along the west boundary of the project will not cause adverse impact to the adjacent properties in the residential neighborhood. 3. In several locations within the SWPPP there are descriptions of goals that are to be attained. Provide descriptions of specific activities or policies that will be followed to achieve these goals. 4. Part IV.B.2.c Explicitly indicate in the SWPPP the name of the operator with operational control over project specifications (including the ability to make modifications in specifications). 5. Part IV.B.2.c Explicitly indicate in the SWPPP the name of the operator with operational control over day-to-day activities at the construction site. 6. Part IV.C.2.b. Describe the intended sequence of disturbance activities 7. Part IV.C.2.d. Estimate the pre-construction and post-construction runoff coefficient and provide soil data and any existent data on the quality of the discharge 8. Part IV.C.2.e. Include a general location map (e.g. U.S.G.S. quadrangle, portion of a city or county map) showing 1 mile radius around site 9. Part IV.C.2.e. Describe record keeping efforts, include forms/checklists used for keeping the required data 20. Part IV.D.4.c.i. Maintain records of the dates when major grading activities occurred 21. Part IV.D.4.c.ii. Maintain records of when construction activities cease (temporarily or permanently) 22. Part IV.D.4.c.iii. Maintain records of when stabilization is initiated and completed and any reason for delays 23. Part IV.D.5. Describe structural practices used to divert flows from exposed soils, store flows and limit runoff and the discharge of pollutants from exposed areas to degree attainable. Part IV.H.4. Describe all areas to be inspected including all disturbed areas of site, and areas used to store materials exposed to precipitation Describe inspection procedures how inspectors will look for evidence of, or potential for, pollutants entering drainage system 38. Describe how inspectors will document all findings and what the inspection form will look like 39. Part IV.H.5. Describe how inspectors will complete an inspection report for each inspection that includes inspection date; name, title and qualifications of each qualified person making the inspection; weather information for the period since the last inspection; location of discharges of sediment or other pollutants; list of BMPs that need to be maintained, failed to operate or prove inadequate; list of additional needed BMPs; corrective actions required; sources of all non-stormwater and control measures; and materials storage areas with evidence of pollutant discharge 40.
