SUMMARY: DSD Engineering Division has reviewed the plan set submittal for Grading Plan review, and does not recommend approval at this time. The following comments apply to the SWPP, Drainage Report, and the civil drawings C1 - C5.

GENERAL COMMENTS:
1) DS Sec.11-01.12: Provide a copy of the Conformatec Geotechnical Report as noted in General Construction Note 34 on sheet C3. Add any specific geotechnical recommendations as general notes if applicable.
2) DS Sec.10-02.2.3: Provide updated drainage report addressing the following:
a) In drainage report, provide explanation and purpose of 18" PVC pipe within basin.
b) Revise exhibits in the drainage report to reflect changes per comments below for sheets C2 and C3.
c) Provide 100-year ponding elevation for basin and add notation on planview.
d) DS Sec.10-02.14.2.6: Provide discussion regarding results from soils report for hydro-collapsing soil potential with any recommendations for setbacks from any building structures.
3) DS Sec.11-01.4.1.C: On some sheets the text, dimensions, and elevations are blurry and illegible; consider revising text thickness.
4) Some of the civil drawing sheets with the same sheet number have different seal dates in a separate set. Assure all sheets match in each set and are the most current print.
5) DS Sec.11-01.2.3.B: A grading bond for native seeding will be waived for this site.

GRADING PLAN SHEETS COMMENTS:
6) DS Sec.11-01.4.1.C.3: Address the following grading comments for sheet C2:
a) Provide datum for basis of elevation.
b) Provide estimates for total disturbance area in square feet as a notation.
c) On the grading plan, show and label delineation for disturbance area and show limits of grading. Provide dimensioning for clarification. Clarify extents of disturbance area needed to construct all portions of the project under this proposed grading permit.
d) Show grading for flow from Pool to Pool Backwash Disposal Basin. Also show flow arrows to indicate proposed direction of flow.
e) For pool area, depict drainage directions with discharge locations noted. Please include reference to splash pads or other energy dissipating type items at any wall opening locations.
f) Sidewalk scuppers are required at any location where site drainage may be concentrated. Show any locations on planview.
g) For legend, add hatching for sidewalk/concrete, and clarify riprap as shown in the legend. Label all areas and specify all riprap dimensioning, thickness, average diameter - specify if to be placed on filter fabric.
h) Clarify dimension of driveway apron keynote 7 on planview. In response letter, explain purpose of this curb opening.
i) On planview, clarify locations of existing pavement as opposed to new proposed areas to be constructed.
j) Provide dimensions for length of proposed channel on planview.
7) DS Sec.11-01.4.1.C.3: Address the following General Construction Note comments for sheet C3:
a) The grading limits shall be shown on planview and these are the limits of disturbance: Revise General Construction Note 9.
b) Revise General Construction Note 19 to include information about BlueStake.
c) Add as a General Construction Note that all grading shall conform to Development Standards Sections 11-01 and 9-04 and the current IBC.
d) Add notation or General Construction Note stating that the location for construction access to site shall be from the designated entrances only.
e) Add as a General Construction Note that a Right-of-way Use Permit may be required for any earthwork construction within the existing public right-of-way.
f) Add as a General Construction Note that any future walls that may need to be constructed require separate building permits.
g) Add as a General Construction Note to call 791-5550 for Final Grading inspection.

8) DS Sec.11-01.4.1.C.3: Address the following grading comments for sheet C4: SWPPP Construction Checklist can be found at: http://www.azdeq.gov/environ/water/permits/download/cswppp.pdf The SWPPP shall be accepted prior to approval of the Grading Plan. The Stormwater Pollution Prevention Plan (SWPPP) does not meet the minimum requirements of the AzPDES Construction General Permit (CGP). Revise the SWPPP to according to these comments:
a) Regarding Operators, on the SWPPP, clarify indication of the name(s) of the party(ies) with day-to-day operational control over project specifications (including the ability to make modifications in specifications) and control of those activities necessary to ensure compliance with the SWPPP or other permit conditions. Provide a table for recording the names and responsibilities for each party responsible for activities necessary to ensure compliance with the SWPPP or other permit conditions. Provide this information on the plan or SWPPP Report. Note that the signatures from the operators must be on the SWPPP on the site copy of the SWPPP at or before the commencement of construction. (Part IV.B.1.d & Part IV.J.1).
b) Revise the General Note regarding receiving waters on sheet C4. Revise Stormwater Pollution Prevention Plan to provide a general location map showing and identifying receiving waters within 1 mile including unnamed watercourses and any storm drains that may receive discharges from the site. (Part IV.C.2.e).
c) Update soils information in General Notes on sheet C4.
d) Revise page 2 to clarify that the clearing and grubbing and demolition will occur after the erosion controls are established. (Part IV.D.4.c.i)
e) Delineate the grading limits on planview. Document areas where existing vegetation will be preserved and state how area will be designated by adding notation for string line or construction fencing. (Part IV.D.4.a).
f) Revise last General Note on sheet C4. Depict on plan view the concrete washout area - this should not be located in the existing basin area or outside of the grading limits. (Part IV.D.7).
g) Clarify construction entrance location - clarify whether the site will be accessed from the existing western parking lot.
h) Add a note stating that if sediment does leave the site, off-site accumulation of sediment must be routinely removed to ensure no adverse effects on water quality. (Part IV.D.2.c).
i) Describe measures to minimize the generation of dust, in addition to sweeping streets, such as the provision for water trucks. (Part IV.D.8.b)
j) Revise cross section for silt fence installation to clarify that there will be no compacted fill blocking the approaching flows. (Part IV.D.1)
k) On planview, show directions of stormwater flow including lot drainage areas (e.g., use arrows to show which ways stormwater will flow) and approximate slopes anticipated after major grading activities. (Part IV.C.3.a)
l) Regarding summary of potential pollution sources, identify the location and describe any stormwater or non-stormwater discharges at the site associated with other pollutant sources such as fueling operations, materials stored on-site, waste piles, etc. Show on plan view material and equipment storage areas as well as locations for clean out area(s).(Part IV.C.5)
m) Specify that all erosion and sediment control measures must be properly selected, installed, and maintained per the manufacturers' specifications and good engineering practices. If periodic inspections or other information is discovered that indicates a control has been used inappropriately, or installed incorrectly, the operator must replace or modify the control for site situations as soon as practicable and before the next anticipated storm event. (Part IV.D.2.b)
n) Describe good housekeeping procedures to prevent litter, construction debris, and construction chemicals exposed to stormwater from becoming a pollutant source for stormwater discharges. List specific waste disposal methods and practices, such as dumpsters. (Part IV.D.3)
o) Thoroughly describe appropriate BMP's to minimize pollutants in any non-stormwater discharges. Superchlorinated wastewaters must be held on-site until the chlorine dissipates, or otherwise dechlorinated prior to discharge, except if used in emergency firefighting. (Part IV.D.7)
p) For inspections, revise/provide example forms for Compliance Evaluation Reports (Inspection Reports). At a minimum the forms must include:
i) The inspection date. (Part IV.H.5.a)
ii) Provide titles besides name and qualifications of person(s) making the inspection. Qualifications must either be on/attached to the report or alternatively, if the SWPPP documents the qualifications of the inspectors by name, that portion of the SWPPP may be referenced. (Part IV.H.5.b)
iii) Weather information for the period since the last inspection (or since commencement of construction activity if the first inspection), including best estimate of the beginning of each storm event, duration of each event, time elapsed since last storm event, and approximate amount of rainfall for each event (in inches) . (Part IV.H.5.c)
iv) Location(s) of discharges of sediment or other pollutants from the site. (Part IV.H.5.d)
v) Location(s) and identification of BMPs that need to be maintained; failed to operate as designed or prove inadequate. (Part IV.H.5.e)
vi) Location(s) where additional BMPs that do not exist at the time of inspection are needed. ((Part IV.H.5.f)
vii) Corrective actions required, including any changes to SWPPP necessary and implementation dates. (Part IV.H.5.g)
viii) Identification of all sources of non-storm water and the associated pollution prevention control measures. (Part IV.H.5.h)
