1. Copy of recorded Lot combination covenant and MS&R covenant to be provided prior to site plan approval.

2. a) A total of 36 vehicle parking spaces are required for the site prior to future right-of-way dedication. A total of 28 vehicle parking spaces are to be provided, which results in a deficiency of eight (8) vehicle parking spaces.
b) A total of 31 vehicle parking spaces are required for the site after right-of-way dedication. The "future site plan" provides a total of 21 vehicle parking spaces, which results in a deficiency of 12 vehicle parking spaces.

3. A five (5) foot wide pedestrian refuge area with a minimum four (4) foot wide concrete sidewalk is required along the entire south and east sides of building 2. There are portions of this area, which do not comply with the minimum width requirements along the entire path. (D.S. 2-08.4.1.B) (D.S. 3-05.2.2.B.1)

4. Since a Board of Adjustment Variance will be applied for, please list as a general note, the case number, date of approval, variances granted, and any conditions imposed.
