Speedway Blvd.) Grading Plan T04BU00189 (First Review) T14S, R15E, Section 06 RESUBMITTAL REQUIRED: Grading Plan, Drainage Report and SWPPP The Grading Plan (GP), Drainage Report (DR) and Storm Water Pollution Prevention Plan (SWPPP) cannot be approved as submitted. Please address the following review comments prior to the next submittal. Grading Plan 1. Please include a response letter to the comments along with the corrected copies with re-submittal. 2. Add note: "Depress all landscaped areas a maximum of 6" for water harvesting" 3. Add the basin(s) maintenance responsibility note per S.M.D.D.F.M. 2.3.1.6 C 1 and 2 to the GP. See attached sheet. 4. Add to grading note: "All grading shall conform to all the provisions, regulations and amendments of the latest edition of the 2000 IBC" 5. List administrative address/site address on plans. 6. List subdivision or Development number. (D02-0041) 7. List both Cut and Fill quantities in cubic yards. If one or both are 0 cy, please list. 8. Show cross a typical x-section of the PAAL. 9. Refuse storage areas built into patio walls will not be serviced per 6-01.2.2.B.2. 10. Detention/Retention: (Please submits DR for comparison). · Match Drainage report. · Must match drainage report. · Inlet/Outlet structural details (Weir design). · Cross-sections and dimension. · Dimension basin and provide spot elevations. · Security barriers are required if ponding depth exceeds 2 feet, if slopes are greater than 4:1 and/or if pedestrian access is adjacent to basin. · Water surface elevations. · Peak discharge at basin's outlet. 11. Show details of all surface and subsurface drainage devices. A Stormwater pollution prevention plan is required. Contact Paul P. Machado at 791-5550 x1193 for additional information. Drainage report: 1. Please submit 2 copies of the revised DR along with the GP for review . The following comments were made during the CDRC review and should be addressed. 2. Percolation tests are required prior to the approval of subsurface infiltration system. The rate of percolation cannot exceed 12 hours. A determination on the sub-surface retention basins to see if they work properly cannot be made until the percolation results are submitted. The percolation results were not found in this submittal. Please submit the percolation results on the next submittal. Per previous comment on the Development plan review. 3. A provision for maintenance must be provided for the subsurface retention basin. Please add the maintenance responsibility note to the Drainage report. My apologies, the Maintenance checklist should also be included in the drainage report. See Attached copy. Stormwater pollution prevention plan: Site Map 1. Show, on the site map, locations of structural and nonstructural controls identified in the SWPPP. (Part IV.C.3.c) 2. Show, on the site map, locations where stabilization practices are expected to occur. (Part IV.C.3.d) 3. Show, on the site map, all material washout areas. 4. Specify that erosion and sediment controls shall be designed to retain sediment on site to the extent practicable. (Part IV.D.2.a) 5. Include a copy of the general permit and the NOI that was or will be submitted to ADEQ. If other agreements with any state, local, or federal agencies exist that would affect the provisions or implementation of the SWPPP, include copies of those agreements. (Part IV.F) Inspections 6. Specify an inspection schedule in the SWPPP. (Part IV.H.1) Choose either of the following: 7. Provide example forms for Compliance Evaluation Reports (Inspection Reports). At a minimum the forms must include: a. The inspection date. (Part IV.H.5.a) b. Name(s) and title(s) and qualifications of person(s) making the inspection. Qualifications must either be on/attached to the report or alternatively, if the SWPPP documents the qualifications of the inspectors by name, that portion of the SWPPP may be referenced. (Part IV.H.5.b) c. Weather information for the period since the last inspection (or since commencement of construction activity if the first inspection), including best estimate of the beginning of each storm event, duration of each event, time elapsed since last storm event, and approximate amount of rainfall for each event (in inches) . (Part IV.H.5.c) d. Location(s) of discharges of sediment or other pollutants from the site. (Part IV.H.5.d) e. Location(s) and identification of BMPs that need to be maintained; failed to operate as designed or prove inadequate. (Part IV.H.5.e) f. Location(s) where additional BMPs that do not exist at the time of inspection are needed. ((Part IV.H.5.f) g. Corrective actions required, including any changes to SWPPP necessary and implementation dates. (Part IV.H.5.g) h. Identification of all sources of non-storm water and the associated pollution prevention control measures. (Part IV.H.5.h) i. Identification of material storage areas and, evidence of or potential for, pollutant discharge from such areas. (Part IV.H.5.i) j. Compliance certification statement. (Part IV.H.6) k. Signature line with certification statement. (Part IV.H.6) Signature Requirements 8. Include a certification and signature for each operator in accordance with Part VII.K. (Part IV.J.1) 9. All NOIs must be signed and certified as follows: a) For a corporation: By a responsible corporate officer. For the purpose of this Part, a responsible corporate officer means: i) A president, secretary, treasurer, or vice-president of the corporation in charge of a principal business function, or any other person who performs similar policy or decision-making functions for the corporation; or ii) The manager of one or more manufacturing, production, or operating facilities, provided, the manager is authorized to make management decisions which govern the operation of the regulated facility including having the explicit or implicit duty of making major capital investment recommendations, and initiating and directing other comprehensive measures to assure long term environmental compliance with environmental laws and regulations; the manager can ensure that the necessary systems are established or actions taken to gather complete and accurate information for permit application requirements; and where authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures. b) For a partnership or sole proprietorship: By a general partner or the proprietor, respectively; or c) For a municipality, state, federal, or other public agency: By either a principal executive officer or ranking elected official. For purposes of this section, a principal executive officer of a Federal agency is the chief executive officer having responsibility for the overall operations of a principal geographic unit of the agency (e.g. Regional Administrators of EPA.). 10. Reports and Other Information: All NOTs, SWPPPs, reports, certifications, or information required by this general permit and other information requested by the ADEQ or authorized representative of the ADEQ shall be signed by a person described in Part VI.K.1 or by a duly authorized representative of that person. A person is a duly authorized representative only if: d) The authorization is made in writing by a person described in Part VII.K.1; e) The authorization specifies either an individual or a position having responsibility for the overall operation of the regulated facility or activity, such as the position of manager, operator, superintendent, or position of equivalent responsibility or an individual or position having overall responsibility for environmental matters for the operator. (A "duly authorized representative" may be either a named individual or any individual occupying a named position.); and f) The signed and dated written authorization is included in the SWPPP and submitted to the ADEQ upon request. 11. Certification. Any person signing a documents under the terms of this permit shall make the following certification: I certify under penalty of law, that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gathered and evaluated the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing violations.
