Not signed, initialed, or dated, not Revision Number. It calls for (3) 3/0 and (1) #6 GND between the meter base and the Service Panel "B". With no OC device on the line side of the Service Panel, the GND, if present, would be sized according to the phase conductors per NEC 250-102 (c) and Table 250-66. Also, since the "Service Panel" would be the location of the disconnect and hence a Main Bonding Jumper per NEC 250-28, the fourth wire in the run should be the Grounded conductor. NEC 250-24 (b) requires the Grounded Conductor to be brought to the Service equipment. Having the GND as well would consist of a parallel path to ground on the load side of that disconnect.
