Saint Mary's Road PROPERTY OWNER: Carondelet St. Mary's Hospital and Health Center 1) Any revisions to the Site Plan shall be shown for a revised Site Plan submittal under T03CM00208-R. 2) The improvements to the structural building for this project are considered a substantial improvement to an existing floodprone structure if the value of this new improvement is 50% or more than the value of the existing structure in present day value. If the project is determined to be a substantial improvement, and, if it is determined that the lowest floor elevation is below the regulatory floor elevation, a design submittal, and review and acceptance of the submittal for floodproofing will be required and shall conform to City of Tucson and FEMA floodplain development requirements. This submittal falls under Floodplain Use Permit T03OT01270, which includes the following conditions: - A qualified appraisal shall be provided to Development Services Department - Engineering Division. The appraisal shall serve as documentation, as required, demonstrating that the value of the improvements (to the building that has the improvements under this permit) is less than 50% of the current value of the existing structure. - The proposed work shall not impose conditions that would increase the flood damage potential to the combined proposed and existing building, by greater than 50 percent of the existing structure's value. - Any future permanent, freestanding walls shall have separate floodplain use permit applications submitted to this office for review and approval. - Upon project completion, provide one copy of as-built grading plans with a copy of the substantial completion letter from the project engineer stating that there will be no adverse impact to adjacent property, either upstream or downstream as a result of this development, and that the project has been built in conformance to the City of Tucson and FEMA regulations, with acknowledgment that there may be risks depending on response to FEMA's submittal. These grading as-builts may be submitted after addressing any FEMA responses. 3) ALL new St Mary's submittals for 2005 shall be submitted with a Floodplain Use Permit application.
