RESUBMITTAL REQUIRED: Grading plan, Drainage Report and SWPPP.

The Grading Plan (GP), Drainage Report (DP) and SWPPP (Stormwater Prevention pollution plan) cannot be approved as submitted. Please address the following review comments prior to the next submittal.

Grading Plan:
1. Please include a response letter to the comments along with the corrected copies with re-submittal.
2. Please include a detail for the opening of the masonry wall opening for the screen wall the crosses the drainage swale at the North side of the property.
3. Due to the commercial vehicles entering the site, curb returns will be required. A permit or a private improvement agreement will be necessary for any work performed within the Right-of-way. Contact Permits and Codes at (520) 791-5100 for permit information. Please eliminate the driveway apron and install curb radii per D.S. 3-01.3.2.C. Vertical curb is required in the R/W. Per previous comment.

5. Detention/Retention basins must be graded to drain to the basin side toward the weir. This does not necessarily mean to drain out of the basin, but to pond in one section of the basin only. Please add percentage slope to basin.
6. All dimensions for the basin must be shown on the grading plans. Field inspectors do not have copies of the drainage report to confirm the dimensions in the field. Per previous comment.






Drainage Report:
1. The DR is accepted conditionally.
2. Please size the opening of the masonry wall opening for the screen wall that crosses the drainage swale at the North side of the property.
3. COT/DSD Engineering/Floodplain does not accept addendum's to accompany a DR. All additional information must be incorporated into a revised DR.

Signature Requirements
9. Include a certification and signature for each operator in accordance with Part VII.K. (Part IV.J.1)
10. Reports and Other Information: All NOTs, SWPPPs, reports, certifications, or information required by this general permit and other information requested by the ADEQ or authorized representative of the ADEQ shall be signed by a person described in Part VI.K.1 or by a duly authorized representative of that person. A person is a duly authorized representative only if:
a) The authorization is made in writing by a person described in Part VII.K.1;
b) The authorization specifies either an individual or a position having responsibility for the overall operation of the regulated facility or activity, such as the position of manager, operator, superintendent, or position of equivalent responsibility or an individual or position having overall responsibility for environmental matters for the operator. (A "duly authorized representative" may be either a named individual or any individual occupying a named position.); and
c) The signed and dated written authorization is included in the SWPPP and submitted to the ADEQ upon request.
11. Please sign or indicate why the certifications I the SWPPP were not signed.
12. Certification. Any person signing a documents under the terms of this permit shall make the following certification:
I certify under penalty of law, that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gathered and evaluated the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing violations.
