The following items must be revised or added to the Storm Water Pollution Prevention Plan (SWPPP). Please include a response letter with the next submittal that states how all comments have been addressed.

RESUBMITTAL REQUIRED: STORM WATER POLLUTION PREVENTION PLAN.

1. Include in the title block on the Storm Water Pollution Prevention Plan SWPPP, "Site Map".

2. Erosion and sediment controls shall be designed to retain sediment on site. (Part IV.D.2.a) If sediment does leave the site, off-site accumulation of sediment must be routinely removed to ensure no adverse effects on water quality. Address clean up for tracking soil/dirt onto Alvernon Way and Glenn Road if the stabilized construction entrance fails. (Part IV.D.2.c)

3. In areas where construction has temporarily or permanently ceased, stabilization measures must be implemented within 14 days. (Part IV.D.4.b) If construction will resume within 14 days, then no stabilization measures are needed. (Part IV.D.4.b.i) Please include this in the section V, "General Sequence For Implementation of Best Management Practices".

4. Remove sediment from sediment controls when design capacity has been reduced by 50%. Clearly indicate in the section VI, "Structural Erosion Control Best Management Practices", that when the sediment basin's design capacity has been reduced by 50% from sediment accumulation, that the basin will be properly cleaned out. (Part IV.E.3)

5. Show the location on the site map the sediment basin addressed in section VI, "Structural Erosion Best Management Practices". (Part IV.C.3.c.)

6. Include a statement that the project is in compliance with all applicable Federal State, or local requirements. (Part IV.G)

7. Include in the Spill Control & Response section and on the Spill Response form, a general note stating, "Report oil or hazardous materials releases in excess of reportable quantities, or that may endanger human health or the environment, within 24 hours to ADEQ: (602) 771-4466 FAX (602) 771-4505." (Part VI.L.5.a.)

8. Include landscaping, decomposed granite and hydroseeding (if used on site) with the permanent stabilization practices in the implementation schedule. (Part IV.D.4.a)
