The Stormwater Pollution Prevention Plan does not meet the requirements of the February 28, 2003 AzPDES Construction General Permit.
Information on the general permit can be obtained from ADEQ at http://www.adeq.state.az.us/environ/water/permits/stormwater.html.
Revise the SWPPP to be consistent with the following comments and submit three copies for review. All references are to the general permit. Also submit three copies of the Notices of Intent for each known operator. As additional operators become known, they must also submit copies of the NOI to the City of Tucson.
Each operator is responsible for submitting his own NOI and NOT to ADEQ. Forms must be as provided by ADEQ.

Revise the SWPPP to provide the following:
1. Identification of all operators and the areas over which each has control (Part IV.C.1) Leave space to add operators when they become known later.
2. Existing data on soils and quality of site discharges (Part IV.C.2.d)
3. General location map showing receiving waters within 1 mile. (Part IV.C.2.e) This should include Stone Avenue Wash.
4. A copy of the AzPDES permit and the NOI on forms provided by ADEQ. (Part IV.F)
5.The name of nearest receiving waters that will receive discharges from disturbed areas of the project. (Part IV.C.4)
6. Location of construction and waste materials expected to be stored on-site with updates as appropriate. (Part IV.D.8.c) If the locations are not yet know provide a means for updating this information as it becomes available.
7. Description of the spill prevention and control plan. (Part IV.D.8.c)
8. Description of BMPs for nonstorm discharges (Part IV.D.7)
9. Description of vehicle and equipment maintenance controls (Part IV.c.5)
10. For control measures, identification of the operator responsible, (Part IV.D.1)and why BMPs were selected. (Part IV.D.2.b)
11. A statement that all BMPs must be properly selected, installed and maintained per specifications and good engineering practices. (Part IV.D.2.b)
12. A statement that if a BMP has been used inappropriately, it must be must replaced or modified within 24 hours. (Part IV.D.2.b)
13. Erosion and sediment controls that have been designed to retain sediment on site. (Part IV.D.2.a) If sediment does leave the site, off-site accumulation of sediment must be routinely removed. (Part IV.D.2.c)
14. Provision that sediment will be removed from sediment controls when design capacity has been reduced by 50%.(Part IV.E.3)
15. Placement of velocity dissipation devices at discharge locations and along the length of any outfall channel to provide a non-erosive flow velocity. (Part IV.D.5.b) Describe the timing for such placement.
16. Locations on the site plan where stabilization practices are planned.
17. Description of the inspection requirements outlined in Part IV.H. Include selection of inspection schedule or provide descriptions of the available alternative schedules with instructions for indicating which has been selected by the operator.
