T03BU00002 Reviewed by: Loren Makus The Stormwater Pollution Prevention Plan does not meet the Requirements of the Arizona Pollutant Discharge Elimination System Construction General Permit. The permit can be obtained from the ADEQ web site:http://www.adeq.state.az.us/environ/water/permits/stormwater.html. Note: All citations below refer to the AzPDES permit. Please provide the following: 1. Three copies of the signed copy of Notice of Intent (NOI) for each known operator. The NOI must be on the form provided by ADEQ. (Part III.F) (Each operator/permitee must submit an NOI to ADEQ as well) Revise the SWPPP to provide for all of the following: 2. Identification of all operators and the areas over which each has control.(Part IV.C.1) 3. Signed operator certifications for each known operator. (Part IV.J.1) 4. Intended sequence of soil disturbing activities (Part IV.C.2.b) 5. Estimated run-off coefficients for pre-construction and post-construction conditions (Part IV.C.2.d) 6. Identify location of construction and waste materials expected to be stored on-site with updates as appropriate. (Part IV.D.8.c) 7. List sources of nonstormwater discharges including water from dust control, vehicle washing, and concrete washout. (Part IV.D.7) Concrete, drywall, paint and stucco washout must not be allowed into any paved street or into any drainageway. 8. Include a copy of the permit and the NOI. (Part IV.F) These must be the documents as provided by ADEQ. 9. Describe BMPs for each major activity in the project description; for on and off-site construction, waste and material storage areas or support activities. (Part IV.d.1) 10. Describe spill prevention and control plan (Part IV.D.8.c) 11. Describe BMPs for nonstorm discharges (Part IV.D.7) 12. For control measures, identify the implementation sequence and the operator responsible, (Part IV.D.1)and why BMPs were selected. (Part IV.D.2.b) 13. All BMPs must be properly selected, installed and maintained per specifications and good engineering practices. (Part IV.D.2.b) 14. If a BMP has been used inappropriately, it must be must replaced or modified within 24 hours. (Part IV.D.2.b) 15. Erosion and sediment controls shall be designed to retain sediment on site. (Part IV.D.2.a)If sediment does leave the site, off-site accumulation of sediment must be routinely removed to ensure no adverse effects on water quality. (Part IV.D.2.c) 16. Describe good housekeeping practices to prevent litter, construction debris, and construction chemicals from leaving the site (Part IV.D.3) 17. Include statement that the project is in compliance with all applicable federal State, or local requirements. (Part IV.G) 18. In areas where construction has temporarily or permanently ceased, stabilization measures must be implemented within 14 days. (Part IV.D.4.b) If construction will resume within 14 days, then no stabilization measures are needed. (Part IV.D.4.b.i) 19. Describe interim and permanent stabilization practices, including an implementation schedule. (Part IV.D.4.a) 20. The following records of stabilization must be maintained: Dates when major grading activities occur. (Part IV.D.4.c.i) Dates when construction temporarily or permanently ceases on a portion of the site. (Part IV.D.4.c.ii) Dates when stabilization measures are initiated and completed. (Part IV.D.4.c.iii) 21. The drainage report identifies Concentration Point 4 with a drainage area of 40.2 acres. On the SWPPP site diagram, show the location of CP-4. Address the requirements of the permit for this drainage area: For drainage areas of 10 or more acres, provide temporary or permanent sediment basin with storage for calculated volume of runoff from a 2 year, 24 hour storm. (Part IV.D.5.a.i) or In determining drainage area, do not consider runoff from undisturbed or stabilized areas and off-site flows that are diverted around the disturbed area and the sediment basin. (Part IV.D.5.a.i) or Where runoff volume is not calculated, provide temporary or permanent sediment basin with a storage volume of 3,600 cubic feet of runoff per acre. (Part IV.D.5.a.i) or If a sediment basin with these volumes is not achievable, then smaller sediment basins and or sediment traps shall be used. (Part IV.D.5.a.ii) If non-attainability is claimed, include an explanation. (Part IV.D.5.a.i) 22. Place velocity dissipation devices at discharge locations and along the length of any outfall channel to provide a non-erosive flow velocity.(Part IV.D.5.b) 23. Describe how soil will be stabilized at channel outlet to prevent rills and gullies. (Part IV.D.8.e) and (Part IV.D.6) On Plan Sheets or Site Map show: 24. Drainage patterns, (Part IV.C.3.a) 25. Approximate slopes after major grading, (Part IV.C.3.a) 26. Areas of soil disturbance/Areas which will not be disturbed, (Part IV.C.3.b) Clearly delineate the area around the existing residence that will not be disturbed. 27. Locations where stabilization practices are planned, (Part IV.C.3.d) 28.Describe BMPs to minimize the generation of dust. (Part IV.D.8.b) 29 Describe inspection requirements (Part IV.H.1) Specify the inspection schedule either: once per week, OR once per 14 days and within 24 hours of a 0.5” rainfall. Inspection can be reduced to monthly if, site has been temporarily stabilized, OR Construction is in an arid area, AND construction is occurring during the seasonal arid period. Monthly schedule must also include pre and post storm inspections. Submit three copies of the revised SWPPP for review. |
