1) Carry the 1/0 equipment ground from the 400 amp fused switch to the transformer and bond to the transformer enclosure. In addition include a grounding electrode system in the 400 amp fused switch, connect it to the grounded conductor (neutral) of the service lateral and bond both to the 400 amp fused switch enclosure.
The secondary feeder of the 100 kVA transformer serving the 400 amp circuit breaker should not include an equipment ground. Do not bond the neutral to ground in the transformer.
Bond the neutral to the grounding electrode system in the 400 amp circuit breaker and bond both to the switch enclosure.
This grounding scheme will prevent the possibility of normal neutral currents on the equipment grounding conductors. See originally submitted blue line documents, I have redlined the requirements on the plan.

2) Circuit A-31 on page E-04 serves 12 receptacles at 180VA each, therefore the load in the panel on page E-01 should be 2160VA minimum. In addition the total load exceeds the rating of the 15 amp circuit breaker. See 99NEC 220-3(b)(9).
3) It appears the circuit breakers serving the AC's should be reduced to 50 amps.
GENERAL COMMENTS
1) Provide a disconnect within site of each of the AC's or provide a circuit breaker capable of being locked in the open (off) position. See 99NEC 440-14 ex 1
