Letter of Intent from the Contractor: Includes the project title, project address, Activity Number, and Arizona ROC number with CR-2 or A-5 grading license classification. https://roc.az.gov/contractor-search 3. Payment receipt (if paid within the past 1-2 days). If longer than that, no receipt needed.
