Plans show 2 power poles, but there are 3 existing poles on site. Guy wire/anchor may be in conflict. Proposed location for source of power is not available. Overhead lines on East side of parcel may be a viable option for power. Enclosed is a copy of a TEP facilities map showing the approximate location of the existing facilities. Any relocation costs will be billable to the customer. In order to apply for electric service, call the New Construction Department at (520) 918-8300. All improvements and site information, such as adjacent rights-of-way and property lines, shown on the landscape plan will be identical in size and location to those shown on the base plan (site plan or tentative plat). Should amendments be required to the base plan through the review process, the same amendments will be made to the landscape plan which will then be resubmitted along with the base plan. The landscape plan will contain the following identification in the lower right corner of each sheet: Any relevant case numbers for reviews or modifications that affect the site. ARTICLE 7: DEVELOPMENT STANDARDS 7.6. LANDSCAPING AND SCREENING Indicate graphically compliance with conditions of rezoning Case # C9-15-03. Section 3.3.3.G.5.c UDC, An applicant has one year from the date of application to obtain approval of a site plan that complies with zoning and other development requirements in effect at the time of application, unless an ordinance adopted by Mayor and Council during this period states otherwise. A site plan application that has been in review for a period of one year and has not yet been approved is considered denied. To continue the review of a site plan for the property, a new site plan must be submitted that complies with regulations in effect at the time of re-submittal. The new submittal initiates a new one-year review period. One year Expiration date is September 13, 2019. SECTION 2-06.0.0: DEVELOPMENT PACKAGE (TENTATIVE PLATS AND SITE PLANS) Section 2-06.3.0 FORMAT REQUIREMENTS 2-06.3.5 - A three-inch by five-inch space shall be reserved in the lower right quadrant of each sheet for an approval stamp. 1. COMMENT: Remove the Development Package PDSD approval stamp on all sheets. Reserve a three-inch by five-inch space in the lower right quadrant of each sheet for an approval stamp. The approval stamp will be applied electronically. CONTENT REQUIREMENTS 2-06.4.1 - The name, mailing and email addresses, and phone number of the primary property owner of the site, the developer of the project, registrant(s), and other person(s), firm(s), or organization(s) that prepared the development package documents shall be provided on the right half of the cover sheet. The applicable registration or license number shall be provided if prepared by or with the assistance of a registered professional, such as a surveyor, architect, landscape architect, or engineer. All sealing shall be consistent with Arizona Board of Technical Registration guidelines. 2. COMMENT: List the Landscape Architects information on the cover sheet as noted by the standard above. 2-06.4.2 - The title block shall include the following information and be provided on each sheet: 2-06.4.2.B - A brief legal description and a statement as to whether the project is a re-subdivision are to be provided. On re-subdivisions, provide the recording information of the existing subdivision plat; 3. COMMENT: Add a brief legal description of the subject development parcels in the title block of all plan sheets. 2-06.4.3 - The administrative street address and relevant case numbers (development package document, subdivision, rezoning, board of adjustment, DDO, MDR, DSMR, overlay, etc.) shall be provided adjacent to the title block on each sheet. 4. COMMENT: Provide the development package case number, DP17-0, adjacent to the title block on each sheet. 2-06.4.7 - General Notes The following general notes are required. Additional notes specific to each plan are required where applicable. 2-06.4.7.A - Zoning and Land Use Notes 2-06.4.7.A.3 - If the plan/plat has been prepared in conjunction with a rezoning application, add the following note next to the existing zoning note: "Proposed zoning is ____." List the applicable rezoning file number and conditions of approval. Also place the C9-__-__ (if applicable) and the plan/plat file numbers in the lower right corner of each sheet. 5. COMMENT: Add the following under general note 2 of the cover sheet; "Development of this parcel is subject to compliance with Rezoning case C9-15-03, see sheet 2 for rezoning conditions." 2-06.4.7.A.4 - Identify the existing and proposed use of the property as classified per the UDC. List all UDC sections applicable to the proposed uses. 6. COMMENT: Revise general note 3 to include the correct use specific standards for each use; Medical Service: Major - Subject to UDC 4.9.13.O and Medical Service: Outpatient - Subject to UDC 4.9.4.O.2 and 4.9.13.) If the uses listed by me in this comment are incorrect, please list on the DP cover sheet the correct uses including the applicable use specific standard for each use. 2-06.4.7.A.6 - If a plan or plat is prepared in conjunction with other applications or overlays or the parcel being developed is subject to conditions of an application processed previously, additional information must be added to the plan. Such applications and overlays include, but are not limited to: annexations; rezonings; special exceptions; Board of Adjustment variances; Design Development Options; Technical Standard Modification Request; overlays (Airport Environs Zone, Environmental Resource Zone, Gateway Corridor Zone, Hillside Development Zone, Historic Preservation Zone, Major Streets and Routes, Rio Nuevo District, Scenic Corridor Zone, WASH); Modification of Development Regulations through the Downtown Area Infill Incentive District or Rio Nuevo District; Downtown Heritage Incentive Zone; or, Design Review Board. Provide the following information on the plan. 7. COMMENT: Add a general note on the cover sheet that this development has been designed to comply with Development criteria of the Major Streets and Routes UDC Section 5.4. 2-06.4.7.A.8 - For development package documents provide: 2-06.4.7.A.8.a - Floor area for each building; 8. COMMENT: Include under general note 3the square footage proposed for each use. 2-06.4.7.A.8.d - When the proposed site is part of a larger site, the calculations encompass the entire site, whether existing or proposed. If the project is being phased, calculations must show that, at each phase, requirements are being met. 9. COMMENT: Clarify if this will be a phased development. Per the PDP dated 6/29/18 a second building at the northeast corner of the development site is proposed and a future parking area is proposed south of the northeast area. The PDP is not clear either way. However on this DP there is a note for a "Future Parcel B" and no indication of a second building or development or any reference to a future parking area. 2-06.4.8.A - Provide site boundary/subdivision perimeter information, including bearing in degrees, minutes, and seconds, with basis for bearing noted, together with distances in feet, to hundredths of a foot, or other functional reference system. 10. COMMENT: The site boundary distance and bearings could not be verified on the plans. Add the information as noted by the standard above and include any distance and bearing for the new parcel if it is to be create as part of the this development. 2-06.4.8.B - All easements shall be drawn on the plan. The recordation information, location, width, and purpose of all easements on site will be stated. Blanket easements should be listed in the notes, together with recordation data and their proposed status. Should an easement not be in use and be proposed for vacation or have been abandoned, so indicate. However, should the easement be in conflict with any proposed building location, vacation of the easement shall occur prior to approval of plan unless written permission from easement holder(s) is provided. 2-06.4.9.L - All proposed easements (utility, sewer, drainage, access, etc.) are to be dimensioned and labeled as to their purposes and whether they will be public or private. The easements may have to be recorded and the recordation information added to the development package prior to approval. 11. COMMENT: If applicable to this development site, provide the easement information as noted by the two standards above. 2-06.4.8.C - The following information regarding existing private or public right-of-way adjacent to or within the site shall be provided: the name, right-of-way width, recordation data, type and dimensioned width of paving, curbs, curb cuts, and sidewalks. 12. COMMENT: The information noted in the standard above shall be added to the sheets 4 and 5. Add all missing information. The sidewalks along the street frontage shall be a minimum of 6 feet wide and per rezoning condition 24. Broadway and Rosemont are considered major streets on the MS&R map. 2-06.4.9.A - Draw in all proposed lot lines with approximate distances and measurements. 13. COMMENT: The parcel tax combo must be completed prior to approval of the DP. Include the approval documents with the next submittal. Will that include the creation of the northeast parcel? Also, the note related to a future parcel is labeled on the cover sheet. When is this parcel to be created? It may also be better to add a note that states when the parcel will be created such at time of the new development or lot reconfiguration or split will be processed under separate Lot Split process etc. 2-06.4.9.E - Proposed land splits or existing lot lines shall be drawn on the plan with dimensions and the identification number and approximate square footage of each lot. (Please be aware that, if land division occurs and the number of lots falls within the definition of subdivision, a subdivision plat is required.) Land splits require a separate permit and review. 14. COMMENT: Add the information as noted by the standard above. 2-06.4.9.F - All existing zoning classifications on and adjacent to the project (including across any adjacent right-of-way) shall be indicated on the drawing with zoning boundaries clearly defined. If the property is being rezoned, use those boundaries and classifications. The basis for this requirement is that some zoning requirements on a project are based on the zoning classification of adjacent property. Also, in some instances, each zone has to be taken into consideration on property that is split by two or more zoning classifications, as each may have different requirements. 15. COMMENT: Label all zoning classifications adjacent to the development site including across the streets and alleys. 2-06.4.9.G - If the project is to be phased, provide calculations, setbacks, etc., to indicate that each phase complies with all requirements as a separate entity. Show phase lines on the drawing. Show and label any temporary improvements that may be needed to make the site function for each phase as one entity. If such temporary improvements are off the site of the phase under consideration, a temporary easement or other legal documentation to assure legal use of the property is required. Note recording information. 16. COMMENT: See related comment 9 above. 2-06.4.9.H.4 - Indicate if existing streets are public or private; provide street names, widths, curbs, sidewalks, and utility locations, all fully dimensioned. 17. COMMENT: Provide any missing information as noted by the standard above. See related comment 12 above. 2-06.4.9.H.5.d - Show bicycle parking facilities fully dimensioned. For specifics, refer to Section 7.4.9, Bicycle Parking Design Criteria, of the UDC. Provide, as a note, calculations for short and long term bicycle spaces required and provided. 18. COMMENT: Include a dimensioned detail drawing of the long term bicycle parking facilities. 2-06.4.9.O - All applicable building setback lines, such as erosion hazard, floodplain detention/retention basins, and zoning, including sight visibility triangles, will be shown. 19. COMMENT: Draw and label the required and proposed street building setback from the future back of curb location along Broadway. 2-06.4.9.Q - Provide the square footage and the height of each commercial, industrial, or business structure and the specific use proposed within the footprint of the building(s). 20. COMMENT: Revise the annotation within the building footprint to state just "Medical Service Major: Micro Hospital and Out Patient". Remove the reference to the existing uses. 2-06.4.9.S - Show existing or proposed pedestrian circulation along abutting rights-of-way. Such sidewalks must comply with accessibility requirements for the physically disabled and the design criteria in Section 10-01.0.0, Street Technical Standards, of the Technical Standards Manual. 21. COMMENT: This development is considered one site and the sidewalk requirements may have to be provided along the entire street frontage on Broadway and Rosemont. See the engineering review comments related to this comment. 2-06.4.9.T - Show refuse collection areas, including locations of dumpsters, screening location and materials, and vehicle maneuverability, fully dimensioned, and access route. If dumpster service is not proposed, indicate type of service. For specific information on refuse collection, refer to Section 8-01.0.0, Solid Waste and Recycle Disposal, Collection, and Storage, of the Technical Standards Manual. Refuse collection on all projects shall be designed based on that section, even if collection is to be contracted to a private firm. 22. COMMENT: The keynote symbol or key note for the dumpsters could not be verified on sheet 9. Add the as required. 2-06.4.9.U - Indicate graphically, where possible, compliance with conditions of rezoning. 23. COMMENT: Include a separate response memo that states how all rezoning conditions have been addressed on the DP. Demonstrate compliance with the rezoning conditions on the DP where possible. 24.
